This is an excellent opportunity to develop the Children and Young People’s Social Prescribing offer within Cranstoun. Link workers working closely with GP’s offering social prescribing to young people across the Dudley borough. Social prescribing empowers people to take control of their health and wellbeing offering non-clinical interventions and giving time and focus on ‘what matters to me’ taking a holistic approach, connecting people to diverse community groups and statutory services for practical and emotional support.
This exciting role will be part of Cranstoun Dudley which is the integrated young people’s service in Dudley. As part of our Dudley offer Cranstoun Children and Young People’s Youth and Family Social Prescribing Service is a free, voluntary, non‑clinical service for young people aged 11–25 and their families who are registered with a GP within one of the six Primary Care Networks (PCNs) in Dudley.
The purpose of the service is to provide early, preventative and proportionate support by addressing the social, emotional and practical factors that impact a young person’s health and wellbeing. The service is designed for young people experiencing emerging or low‑level needs, including (but not limited to) stress, low mood, anxiety, isolation, reduced confidence, lifestyle pressures, and practical challenges such as transport, finances or access to opportunities.
The right candidate will be confident in navigating community links to find new opportunities for children and young people to engage in wellbeing boosting activities.
