We’re looking for a compassionate, proactive registered nurse to join the Phyllis Tuckwell Hospice at Home Team. You’ll deliver the highest standards of palliative and end of life nursing care to patients, their families and carers in the community—providing holistic assessment, identifying needs, planning care and delivering timely interventions in partnership with Phyllis Tuckwell multidisciplinary team and colleagues across health and social care.

This role requires a strong commitment to enabling choice and providing respectful, dignified support for people as they approach the end of life, and for those close to them. You’ll contribute to a safe, effective and efficient culture by participating in risk assessment and working in line with PT policies and procedures—helping to ensure a seamless experience for every patient.

About You:
A successfulRegistered Nursewill have:
  • RGN Degree Level Qualification
  • Current NMC registration in adult or general nursing
  • Extensive palliative/end of life care or long-term conditions experience
  • Car driver and owner
  • Experience of multidisciplinary team working and external partners
  • Able to make autonomous decisions demonstrating decision-making, critical thinking and proactive caseload management
  • Excellent communication and interpersonal skills
  • An understanding of community working and Phyllis Tuckwell’s philosophy and values (we will be Compassionate, Inclusive, Dynamic and Accountable)
  • Self-awareness and effective coping strategies
