Main Purpose:

We have an exciting opportunity coming up in the Adult Services team at Dorset Mind. We are looking for Active Monitoring Practitioners to work in a Primary Care Network in Dorset to deliver early intervention for people over the age of 18 experiencing mild to moderate mental health problems.

Active Monitoring is a programme of supportive intervention and guided self-help to improve mental health and wellbeing. Active Monitoring Practitioners offer a series of interventions over a six-week period. They provide support through pre-designed guided psycho-educational programmes. We offer a number of pathways for clients to choose from, that will guide their progress throughout your sessions. These pathways include anxiety and panic attacks, low mood and depression, managing stress, managing anger, low self-esteem, managing isolation, and grief and loss and Menopause. Active Monitoring Practitioners will help clients to select the one that is most appropriate for them.

Two positions are available, both for 20 hours per week.

We have services that run in partnership with the following medical groups: Bournemouth Central PCN, Poole Central PCN, North Poole PCN, North Bournemouth PCN, Purbeck PCN, and Weymouth and Portland PCN.

All training will be provided.

Job Title: Part-time Triage Practitioner (multiple roles)
Responsible to: Team Leader
Hours: two roles available – 20 hours and 17.5 hours
Pay Scale: B3 – £12.78 per hour

Base:​ ​Remote Working, Within Bournemouth Central PCN or Poole North PCN

Main Purpose:

We are expanding our Mental Health Triage service working within Primary Care Network (PCN) settings in Dorset, so we are looking for a Triage practitioner who is supportive, caring, resilient, solution-focused, and confident.

The role of the Triage team is to offer an enhanced initial triage assessment to patients requiring support with their mental health. The aim is to provide an extended first contact fact-finding appointment for the patient to talk about their needs, and signpost them to suitable, relevant services. This reduces inappropriate referrals to services, which can feel frustrating for patients and frees up GP’s time.

The successful candidate will be offering the triage appointments daily via remote working. These appointments will be for 20 minutes each patient and delivered via the telephone, with up to 10 patients a day.

You will work closely with our existing well-structured services already embedded in the PCNs with specialisms in Adult Active Monitoring and Children and Young People’s Active Monitoring, and other services offered within the PCN like Steps2wellbeing, Help and Care, and local support groups, which all form key pathways from the Triage Assessments.

Two roles are available, 20 hours with Bournemouth Central PCN and 17.5 with Poole North PCN

All training will be provided.
