Community Connector – Team Lead/Manager
Hours: 37 hours per week. Flexible working options available.
Salary: £28,434.00 - £30,754.90 (salary dependent on skills, experience and knowledge)
Contract: Permanent
Base: Berkshire West – Main Base TBC (Travel across the geographical area as required)
The Community Connector Team Manager role is fundamental to the ongoing development of the Community Connector service within the MHICS teams, following on from a successful pilot year. The Team Manager line manages, supports and leads a team of Community Connectors who will support patients to access community services and resources that support maintaining good health and wellbeing.
This includes:
- Information and guidance around emotional and physical wellbeing
- Space and support to help make sense of difficulties
- Practical support to access community resources
- Brief interventions around ways of coping with stress and anxiety
- Access to a mental health pharmacist from Berkshire Healthcare NHS Foundation Trust, to help with medication issues
- Links to other mental health services and providers of therapy
- Links to local services, charities and organisations, such as drug and alcohol services, housing, finance, Citizens Advice
- Support with employment

- Functional Skills Level 2 or GCSE Maths and English 9-4 or equivalent experience.
- Knowledge and understanding of mental health issues and with diverse needs in a professional environment.
- An understanding of good practice in safeguarding Adults.
- Relevant experience of managing a team in a professional environment.
- Experience of working within mental health setting.
- Experience of working in a demanding environment.
- Experience of working as part of a team and communicating effectively.
- Experience of working with individuals or groups, enabling them to identify their own goals to support personal wellbeing and recovery, and helping them to pursue these.
- Ability to work both collaboratively as a team and independently.
- Ability to be an excellent listener and able to make connections with people, utilising interpersonal skills.
- Ability to confidently use IT systems and software at a basic level.
- Good planning and organisational skills.
- Advanced understanding of diversity in local communities and cultural differences.
