Job Summary:
Operating within a small team providing in-reach and out-reach support, the Support Worker will provide high level support to customers and provide specialist advice, guidance and enable customers in their journey to achieve their goals. The Support Worker will provide support services to customers and the wider local population through a number of proactive and reactive activities focused on key outcome themes – improved mental health, reduction of criminal behaviour, substance misuse support including harm minimisation advice and guidance, improved life skills, positive behaviour engagement, independent living.
The Support Worker role will require flexible and adaptive working practises to meet client’s immediate support needs and support requests from other services. The role will be supported by a team of Support Workers and Practitioners with specialist knowledge and skills to assist in meeting a diverse range of customer needs as outlined in customer support plans, and will receive regular case management support to assist customers to achieve their goals and maintain good quality support practice.

Our housing services have helped positively change thousands of lives for nearing 50 years.
Supported housing provides specialist support for people who are vulnerable and at risk or who are in need of urgent accommodation. This includes people who are homeless or at risk of homelessness, and victims of domestic abuse. Individuals are supported to transition into longer-term accommodation and can also get help to access education, employment opportunities and healthcare.
The key responsibilities will require the postholder to work proactively and liaise with customers, building a rapport and being the first point of contact in the building overnight. Shifts are on a lone working basis, and the postholder will work within a wider team to provide a safe, secure and comfortable environment, helping customers begin to recover from the trauma experienced through homelessness.

Drawing on experiences and learnings from decades of assistance to those most vulnerable in our community, STEAM provides clients with a calming and informed environment beneficial to their needs.

Main Responsibilities:
- To work in conjunction with managers and other key agencies in developing both policy and associated practice that meet the combined needs of the client group

Purpose Statement
Access Community Trust provides supported and unsupported accommodation to residents requiring differing levels of help, support and guidance depending on the individual needs of the client. This may mean a simple sign-posting to an appropriate support service or providing intensive support and guidance which is heavily focused on building personal resilience and skills, enabling and empowering individuals to live independently within the community.
Supported Accommodation: Each referred individual should be supported in a way which encourages residents to be empowered and to help build independence. A key part of gaining independence is being able to maintain an ability to live independently in the community and the role will focus on enabling our customer to build the skills and resilience to achieve this.
Unsupported Accommodation: May require a light touch intervention or more in-depth listening and signposting as appropriate. In some cases those in unsupported accommodation may be undergoing a support assessment and will require intensive signposting and assistance.

Principal Responsibilities:
The key responsibilities will require the postholder to oversee Health and Safety and general security of the buildings, its residents and visitors. The postholder will be responsible for the operational day to day running of the building and be responsible for all of the activities and work undertaken onsite with external support and overview being provided by The Service Manager. Key and centre to the role is the support and development of the skills in the team to ensure operational excellence and delivery in line with the KPI’s set by the funder and the core values of the organisation. Regular case reviews and proactive and reflective case management all form a key part in this process along with support and supervision sessions.
The role will ensure proactive client led working practices are constantly part of a view/review process ensuring our services are meeting the needs of our customers. This work will include working alongside LEAF and proactively working to install positive changes following client feedback

Our housing services have helped positively change thousands of lives for 50 years.
Supported housing provides specialist support for people who are vulnerable and at risk or who are in need of urgent accommodation. This includes people who are homeless or at risk of homelessness, and victims of domestic abuse. Individuals are supported to transition into longer-term accommodation and can also get help to access education, employment opportunities and healthcare.
The key responsibilities will require the postholder to work proactively and liaise with customers, building a rapport and being the first point of contact in the building overnight. Shifts are on a lone working basis, and the postholder will work within a wider team to provide a safe, secure and comfortable environment, helping customers begin to recover from the trauma experienced through homelessness.
