Hours: Flexible working hours to meet the needs of the service to include some unsociable hours, including evenings, weekends and bank holidays.

JOB SUMMARY

As a Wellbeing Practitioner, you will work as part of a new community-based service. You will be working on-site delivering community support, working with each other to scope and develop the model. The service is focused on supporting people from 16 years and upwards across Community, Primary and Secondary care pathways.

The role will lead on a holistic assessment, offering emotional and practical support based on the 5 Ways to Wellbeing and developing safety plans, while also supporting engagement with other system partners as part of the Multi-Disciplinary Team (MDT). System partners include - PCN’s (Primary Care Networks), AWP (Avon & Wiltshire Partnership), blue-light services, Oxfordshire Health and Voluntary Sector to promote and deliver services and achieve the model outcomes.

This role will provide the initial introductions to identify the persons needs and use the recovery model to develop self-confidence and move towards the persons identified goals. The Wellbeing Practitioner will work with the person and the CSMHF Peer Support workers to access relevant organisations and agencies to provide a collaborative service.

Operational Duties

You will engage with our internal wellbeing at work process, having honest discussions around your needs and your wellbeing

Supervise Peer Support Workers and volunteers, offer one-to-one support/debrief meetings when needed by the team. To facilitate honest, open conversations about wellbeing at work and facilitate reflective discussion.

Promote the service by linking in with external agencies to build relationships and promote the service offer, coordinate referrals into the service, and liaise and network with relevant statutory and third sector organisations.

Work with the service user to identify their recovery pathway and move towards their goals, maintaining a person centred, flexible approach walking alongside the service user, implementing interventions/safety plans.

Work flexibly to meet the service needs, responding to referrals within in service timeframes.

Ensure ICT systems and data recording systems are used appropriately in line with policies and procedures of Alabaré. Work with colleagues to ensure effective internal systems including but not limited to maintenance of records and GDPR compliance

Carry out work in accordance with policies of Alabaré and service procedures,current legislation and quality standards.

• To ensure the safeguarding procedures are followed.

Attend, and engage in staff meetings, supervision, appraisal, sickness, absence, overtime, leave and training. Assisting your service manager to ensure staffing costs are contained within budget.

• To assist the service manager in implementing the staff rota to ensure a safe staffing level at all times

• To participate in training as appropriate to maintain an awareness of current developments and best practice

• To support the Service Manager to ensure all external reporting requirements are accurately completed within appropriate time scales.

To work closely with the management team to ensure that the performance targets are met. Including collecting and collatting data used for key performance indicators (KPI) and to support outcomes

Support with the induction of new staff, ensuring effective training and supervision during their probationary period.

• To work together with colleagues and Partner Organisations / Key Stakeholders in a respectful and courteous manner.

To undertake other duties consistent with the responsibilities of the post as defined by your Line Manager and the organisation.

To assist with the responsibilty for the Health & Safety of the building, yourself, co-workers and service users. Report identified risks including Health & Safety and complete appropriate paper work. Ensure alarms are set when locking up the building. Report any suspicious activity outside the building when leaving.

• To participate in supervision and appraisals with the manager and to identify personal development and training needs.

• To undertake such training as required promoting the development of skills.

Experience

Experience of working with people with mental health problems.

Experience of line management responsibilities.

Experience and understanding of living with and managing mental health issues or supporting someone in emotional distress, including understanding the barriers to meaningful living.

Experience of working with people from a range of social and economic backgrounds and understand social justice and inequality issues.

Knowledge and understanding

To have a strong understanding of the needs of people with mental health problems.
To have a good understanding of recovery pathways.

A robust understanding of proactive safeguarding procedures.

To have up to date knowledge and awareness of local and national government initiatives in mental health services.

An understanding of the implications of health and safety to this service.

Have an understanding of substance misuse.

Have an understanding of learning difficulties.

An understanding of support and risk management plans.

Particular skills/essential requirements

Information Technology skills, including Windows based word processing, and email.

Confident Telephone manner to enable service user assessment over the phone.

Full driving licence and willing to transport service users in a company vehicle.

Qualification Appropriate qualification in Mental Health or willing to work towards one e.g. Mental Health First Aid.

GCSE Standard of English language. Literacy and numeracy skills or equivalent.
