About this role…

We are looking for a Support Worker to join the European Post Transitional Service Team.

The Thames Reach Outreach teams operate across London and help those who are rough sleeping to find routes away from the street. The teams respond rapidly and work flexibly to reach people. Individuals are assessed to identify their support needs and support plans are created, including work to obtain identification, access health services and refer to other specialist agencies, including those offering immigration support.

We will not accept applications where Artificial Intelligence (AI) has been used to assist with completing the competency questions.

You will:

  • Work within a team to support a caseload of up to 25 European Economic Area (EEA) National at risk of or currently homeless.
  • Complete outreach shifts on the street, in day centres and in emergency accommodations in the North-London area. The office based in Hackney.
  • Work with several partner agencies including immigration advisor, accommodation providers, local outreach teams, health- and mental health support teams and employment support specialists.
  • Utilise a flexible, trauma-informed approach to support clients with a wide range of support needs and barriers to accessing services.
  • Regularly update notes, support plans and outcomes to monitor and report of your work and track case progression.
  • Represent Thames Reach and clients during the communication with external partners and local authorities.

You will:

  • Be experienced in working with, and supporting vulnerable homeless individuals
  • Be able to provide a flexible and responsive service to meet the needs of rough sleepers
  • Be a proficient user of Microsoft Office applications, including databases
  • Show initiative and attention to detail
  • Effective at developing and maintaining working relationship
  • Have an interest in homelessness and a desire to support us to have a positive impact on the lives of homeless people
