Thames Reach runs projects and services that help to end homelessness in and around London. We work directly with homeless people on the streets and in hostels. We work to prevent vulnerable people from becoming homeless. We help formerly homeless people get their lives back on track. About this role…

We are looking for a Senior Practitioner to join the Ealing Move-On Team.

Floating Support services work with people to sustain their housing. We use a strength-based approach to work with people to identify what support they require and how we can address them together or / and in collaboration with others e.g. landlords, Department of Work and pension or mental health services.

We can support people for the duration of their tenancy, or we can provide short term interventions. We can provide resettlement support when a tenancy commences, and we can equip people with the skills to manage their home and goals independently or when they are move on ready and require minimal support.

We will not accept applications where Artificial Intelligence (AI) has been used to assist with completing the competency questions.

You will:

  • Represent the service at internal and external meetings, building effective partnerships and advocating for the needs of clients and the service.
  • Support the team in proactive problem-solving to address challenges, minimise risks and enable positive progress for service users.
  • Provide day-to-day operational support to the manager, ensuring the service runs efficiently and effectively.
  • Monitor outcomes and performance data, producing reports and identifying actions to improve service delivery.
  • Ensure safeguarding concerns, risks and incidents are identified, managed and escalated appropriately and oversee all health safety concern and reports.
You will have:
  • Experience of delivering outcomes and leading service improvements and change initiatives.
  • An excellent communicator, able to build and maintain positive working relationships with internal and external partners.
  • Provides strategic oversight of case management processes, chairing case review meetings and supporting staff to overcome barriers, progress cases and achieve agreed outcomes.
  • Supports the effective performance of the service through analysis of referral trends, promotion of referral opportunities, and actions that maximise service reach and support the delivery of KPI targets.
  • Demonstrates exceptional reliability, professionalism and judgement, with strong organisational skills and the ability to manage multiple priorities effectively.
