We’re looking for a registered nurse with expertise in palliative and end of life care, to join our community team, to provide specialist palliative care to patients and their families and carers in the community.

We’re looking for a compassionate, proactive registered nurse to join the Phyllis Tuckwell Hospice at Home Team. You’ll deliver the highest standards of palliative and end of life nursing care to patients, their families and carers in the community—providing holistic assessment, identifying needs, planning care and delivering timely interventions in partnership with Phyllis Tuckwell multidisciplinary team and colleagues across health and social care. We are excited to offer this opportunity as current post holders have been successful in progressing into more senior roles within Phyllis Tuckwell.

This role requires a strong commitment to enabling choice and providing respectful, dignified support for people as they approach the end of life, and for those close to them. You’ll contribute to a safe, effective and efficient culture by participating in risk assessment and working in line with PT policies and procedures—helping to ensure a seamless experience for every patient.

About You:
A successful Registered Nurse will have:
  • RGN Degree Level Qualification
  • Current NMC registration in adult or general nursing
  • Palliative / end of life care or long-term condition experience.
  • At least one year of working as a registered nurse with demonstrable competencies related to end-of-life nursing care
  • Car driver and owner
  • Experience of multidisciplinary team working and with external partners
  • Able to make autonomous decisions demonstrating decision-making, critical thinking and proactive caseload management
