Salary: £38,298.33 – £46,117.70 plus London allowance depending on experience
Location: Based at Kenton Ambulance Station and St Luke’s Hospice

As part of the requirements of the NWL ICB model of care, this service will be developed to provide support for patients and families 24 hours a day. Therefore, the post holder will be expected to work shift patterns including night shifts as part of the service development.

The Role
At St Luke’s Hospice, we are committed to providing compassionate, high-quality palliative and end-of-life care to people across Harrow and Brent. Our specialist Pall24 service provides 24-hour advice, support and coordination for patients, families, carers and healthcare professionals, helping people receive the care they need, when they need it. Pall24 is a key part of St Luke’s community and specialist palliative care services.
We are looking for an experienced and motivated Registered Nurse to join our growing Pall24 team as a Senior Nurse. This is an exciting opportunity to play a pivotal role within our Integrated Care Coordination Hub, providing specialist palliative care advice, clinical triage and support to patients, carers and healthcare professionals across North West London.
What You’ll Be Doing
As a Pall24 Senior Nurse, you will:
- Deliver expert telephone advice and support to patients, families, carers and health and social care professionals.
- Undertake holistic assessments and clinical triage, ensuring patients receive the most appropriate care and support.
- Support patients to remain at home wherever possible and contribute to avoiding unnecessary hospital admissions.
- Work collaboratively with community, hospice, acute and social care teams to coordinate patient care.
- Provide specialist palliative and end-of-life care advice to healthcare professionals across a range of settings.
- Identify clinical deterioration, safeguarding concerns and escalating needs, taking appropriate action when required.
- Maintain accurate and timely electronic patient records and contribute to high standards of clinical governance.
- Act as a senior clinical resource within the service, promoting excellent standards of care and professional practice.
What We’re Looking For:
- Registered Nurse (NMC) or Registered Paramedic (HCPC)
- Current NMC registration.
- Significant post-registration clinical experience.
- Experience caring for patients with palliative, end-of-life or complex long-term conditions.
- Excellent communication, assessment and clinical decision-making skills.
- The ability to provide reassurance and support during sensitive and emotionally challenging situations.
- A strong commitment to patient-centred care and collaborative working.
- Experience of telephone triage, community nursing, hospice care, urgent care or care coordination would be advantageous.
