£31,000 per annum (pro rata-Actual £22,621.62). Travel expenses can be claimed on top of regular daily commute.

Location – South Staffordshire District.

This programme of work may be rolled out in year two, to other areas in the District, subject to need.

Have you got experience of helping others and a passion for supporting people through information, advice and guidance?

The Living Well Team Leader role is an exciting new post to raise awareness and educate patients on frailty indicators. The role will have a key focus on working with patients who are identified as mildly frail or have predisposed conditions that would exacerbate frailty, to connect with activities, groups, and services in their community.

If you have good local knowledge of community activities and the initiative to find out more, then being a Living Well Team Leader could be the great new role for you.

The role requires strong organisational skills, the ability to manage a caseload effectively, and sound IT skills. You should also be capable of developing and designing service delivery, with prior experience of line management being an advantage.
