Main base: Wombourne Leusire Centre, South Staffordshire

To focus on Locality 5, Wombourne, Himley, Seisdon and Kinver.

This programme of work may be rolled out in year two, to other areas in the District, subject to need.

The Living Well link worker role is an exciting new post to raise awareness and educate patients on frailty indicators. The role will have a key focus on working with patients who are identified as mildly frail or have predisposed conditions that would exacerbate frailty, to connect with activities, groups, and services in their community.

If you have good local knowledge of community activities and the initiative to find out more, then being a Living Well link worker could be the great new role for you.

You will also need to be organised, able to manage a caseload, have sound IT skills, be able to keep good records and ideally have worked with volunteers before.
