Main base: Royal Stoke University Hospital, with frequent travel across Stoke-on-Trent and North Staffordshire

If you have good local knowledge of community activities and a desire to support people as they are discharged from hospital then being a Hospital Discharge Link Worker could be a great new role for you.
You will also need to be organised, able to manage a caseload, have sound IT skills and be able to keep good records. However, we are more interested in your passion and energy to make a difference, as full training will be provided.
