Hospital Discharge Link Worker
12 months fixed term from start date
30 hours per week
Four Days Per Week – Tuesday – Friday
£28,000 (pro rata), £22,702 (Actual)
Main base: Great Wyrley Health Centre, with fequent travel across Southern Staffordshire

If you have good local knowledge of community activities and a desire to support people as they are discharged from hospital then being a Hospital Discharge Link Worker could be a great new role for you
You will also need to be organised, able to manage a caseload, have sound IT skills and be able to keep good records
However, we are more interested in your passion and energy to make a difference, as full training will be provided
