About the role
Are you caring, compassionate and kind and looking to make a difference to people’s lives, while in hospital and on discharge home? Age UK Isle of Wight is seeking a dynamic, creative, and outgoing person to work within the TOCH (transfer of care hub) at St Marys Hospital to support people on discharge from hospital and in localities hub offices, to support with hospital avoidance and once patients discharged home. This will include community work.
The patients are valued, and they all have a story to tell. You will have a genuine interest and experience of helping older people, have lots of initiative, good organisational skills, and work as part of a team and alone, along with being able to work alone in the community.
The role will be based community locality hub and St Marys hospital to cover holiday on a rota. To establish if any support may be needed when patients go home, and support in the community to avoid hospital admittance. You will be working alongside community nurses. This could be with welfare visits, collection or delivery of prescription, light meal prep, shopping, and light domestic chores for a short period of time, along with welfare phone calls and signposting to community groups. To deliver the community-based support, your own transport is essential and available every working day. Milage will be paid for community visits. You are responsible for accurate record keeping, daily on electronic data bases.

This role involves regulated activity with adults and is subject to an enhanced plus barring DBS check. Anyone on the Adults’ Barred List is legally barred from the role. The successful candidate will undertake a DBS check and any offer of employment made is conditional subject to the assessment of their criminal record (and other pre-employment checks).
