The Prospect@Home team delivers a service that helps people who are living with a life-limiting illness stay in their own homes, if they wish to do so, as they approach the end of their lives. In this rewarding role, you will make a positive difference to people with end of life care needs in their own homes in your local area.
Hours: The role will be a combination of days and nights. Various hours available, including part-time and full-time opportunities.
What is the role?
As a healthcare assistant in our Prospect@Home team you will deliver an exceptionally high standard of end-of-life care and emotional support, day and night, to patients and their families. You will provide person-centred care, including providing planned and urgently required personal care and emotional support. Dependent on your skills and competencies, some more skilled clinical tasks may be required to be performed in patient’s homes.
You will have the support of an amazing team with opportunities to build and develop your career including educational courses to develop your skills.
The healthcare assistant role requires independent working in the community; you must be adaptable, reliable and highly organised in order to prioritise your workload effectively. Experience of working with people with long terms conditions or approaching the end of their life will be an advantage but not essential.
A demonstrable insight into palliative care is ideal and a sensitive communication style is essential
In this role you will be expected to rotate roles and work shifts on our inpatient unit and work flexibly across both teams as needed.

Do you have?
- Minimum Level 2 in Health and Social Care/Care Certificate
- Relevant healthcare assistant experience
- Good verbal and written communication skills
- The ability to work independently in patients’ own homes
- The ability to plan and manage your own workload and activities
- A good awareness of your own limitations, and able to recognise the need for onward referral when needed
- Good IT skills
- A driving license and access to a car, prepared to travel throughout our defined catchment area (Swindon, Marlborough, Pewsey, Wootton Bassett, Lechlade, Fairford and Shrivenham).
It would also be great if you have:
- Level 3 in Health and Social Care
- End of life care experience
