Health & Wellbeing Coordinator
Join Age UK Merton’s Living Well service and help older adults improve their health, independence and confidence through person-centred support. This role involves speaking with clients, understanding their needs, creating practical action plans and connecting them with the right services across Age UK Merton, health, social care and the wider voluntary sector.
The role would suit someone who is empathetic, organised and confident working with both clients and professional partners. You will support referrals, carry out guided conversations by phone and through home visits, record outcomes, and help promote the Living Well service across Merton. This is a full-time fixed-term role based partly at the Elmwood Centre, partly from home, with regular travel across the borough.
