First Contact Clinical Nurse

Salary: £47,951 - £56,863 pa including HCAs WTE (pro rata for part time)

Hours: 22.5 hours per week (part time)

Salary Band: AfC Band 6

Contract: Permanent

We have an exciting opportunity to join for a First Contact Clinical Nurse to join our friendly and supportive Clinical Hub at Trinity. The main purpose of this role is to follow patient referrals, undertake holistic telephone assessments of patients, their families and/or carers to identify their immediate and anticipated specialist palliative care needs.

Responsibilities of a First Contact Clinical Nurse

  • To play a key role in receiving and managing referrals made to the hospice.
  • To ensure a timely, responsive, and effective service for the benefit of all stakeholders involved in the patient referral.
  • Manage all aspects of the referral system to the hospice – identifying appropriateness, urgency, and level of priority.
  • Work collaboratively with internal and external partners to support the coordination and management of patients referred to the hospice, and ensure care is planned to meet the individualised and dynamic needs of the patients, and those close to them.
  • Use assessments and interventions as opportunities to train, coach and mentor clinical staff and visitors to the hospice about specialist palliative care practice.
  • Toprovide immediate and expert specialist nursing assessment, advice and care working alongside existing care providers, and referring to any new services which will benefit the patient and or their family.
  • Facilitate and complete Advance Care Planning (ACP) discussions, ensuring that patients' wishes are clearly documented and communicated in accordance with legal and ethical standards.

Skills and experience required

  • 1st level Registered Nurse
  • Significant experience of providing direct patient services
  • Experience of maintaining effective inter-disciplinary team working structures and systems
  • Enthusiasm and a flexible attitude to working.
  • Experience of managing challenging scenarios
