Job description
Salary: £31,000 per annum (increasing to £32,000 per annum post successfully passing probation)
Hours: 37.5 hours
Job Type: Permanent – Full time
Location: Hybrid - (working from home and Ealing Office)
Interview Date: 19 and 21 May 2026

About the role
The Family Support Practitioner will work alongside the Family Support Manager to support local managers and teams to strengthen and develop their support to people and their families. This role will provide direct support to individual families as needed.
Duties will include:
- Provide emotional and practical support to families through targeted and, where needed, longer‑term support.
- Reduce carer isolation and increase peer support by connecting families and building networks.
- Support families to understand their rights and available support options.
- Attend assessments and meetings in a supportive role, advocating and negotiating on behalf of families where appropriate.
- Increase teams’ confidence and competence in person‑centred approaches.
- Support families and carers who wish to act as peer mentors or trainers.
About you
To be a Family Support Practitioner at Certitude, the following are essential:
- Experience of supporting families of people with learning disabilities, mental health, or autistic people, including families who have experienced trauma in trying to access the right support for their relative.
- An understanding of the needs of people with learning disabilities including those with complex health needs, additional mental health needs and who have experienced trauma.
- Knowledge of the social care system and the role of voluntary provider organisations
- Excellent organisational and time management skills
- Ability to work closely with people from a variety of backgrounds/professions
