Main purpose of the role:

The overall aim of the EMHADS service is to improve health outcomes and reduce health inequalities experienced by people from ethnically diverse communities accessing hospital services across Wirral.

You will provide advocacy, practical support, information and guidance to patients who experience language, cultural or communication barriers whilst receiving care within Wirral University Teaching Hospital sites.

Working closely with hospital staff, patients, families and partner organisations, you will promote equality of access to healthcare services, support effective communication and contribute towards safe and timely discharge planning.

You will work collaboratively with NHS professionals and community organisations to ensure patients receive appropriate ongoing support following discharge and are connected with relevant health, social care and voluntary sector services.

• The role includes travelling across Wirral hospital sites and occasionally within community settings.

Main responsibilities

• Support patients to understand medical information, treatment options, discharge plans and followup appointments.

• Assist patients in communicating effectively with doctors, nurses, therapists and other healthcare professionals.

• Promote patient choice, dignity, independence and informed decision-making.

• Promote independence and empower patients to access healthcare services confidently.

• Work collaboratively with ward staff, discharge teams and multidisciplinary professionals to support safe and effective discharge planning.

Establish effective working relationships with hospital departments and external partner organisations.

Maintain accurate electronic records, ensuring all information is recorded in accordance with organisational procedures and commissioner requirements.

• Promote the EMHADS service across Wirral University Teaching Hospital sites.

• Maintain accurate case records and produce monitoring information, reports and case studies as required by commissioners.

• Ensure all work is undertaken in accordance with safeguarding procedures, GDPR, confidentiality requirements and WMO policies.

Participate in outreach activities, partnership meetings, supervision, training and continuous professional development.

● Undertake any other duties appropriate to the role.

Specific Duties

Responsibilities

• Compile and submit monthly and quarterly monitoring information, case studies and performance reports in line with funder and commissioner requirements.

• Maintain accurate records and contribute to the achievement of agreed Key Performance Indicators (KPIs) and contractual outcomes.

• Ensure all work is delivered in accordance with WMO policies, NHS guidance, funding agreements and contractual requirements.

• Attend supervision, team meetings and service review meetings, providing updates and reports as requested by the Project Coordinator or CEO.

• Support continuous service improvement by identifying opportunities to improve service delivery and patient outcomes.

Representing Wirral Multicultural Organisation

Represent WMO professionally at internal and external meetings, networking events and partnership forums.

Develop and maintain positive working relationships with NHS partners, hospital departments, Primary Care Networks, statutory agencies, Voluntary, Community and Faith Sector (VCFS) organisations and other stakeholders.

• Promote the EMHADS service to healthcare professionals, partner organisations and local communities.

• Work in accordance with WMO's values, professional standards, policies and procedures at all times.

Positively represent the organisation and contribute to maintaining WMO's reputation as a trusted community partner.

Fluent spoken Bengali with a good command of written and spoken English.V
Ability to speak Urdu and/or Hindi.V
