Can you connect people with the right support when they need it most?

As a

  • Supporting people facing challenges with their health, wellbeing, housing, finances or social connections.
  • Helping people set goals and build the confidence to live more independently.
  • Connecting people with local groups, activities and services that meet their needs.
  • Working with health, social care, housing and voluntary sector partners to coordinate support.
  • Visiting people at home and providing practical support following a hospital stay.
  • Keeping accurate records and monitoring progress using case management systems.
  • Experience supporting people in community development, health, social care, learning support or public health settings.
  • Experience working in a charity, community organisation or public sector service.
  • Strong administration skills and confidence using Microsoft Office and digital systems.
  • The ability to learn and use databases and case management systems quickly and accurately.
  • A team-focused approach and the ability to build positive working relationships.
  • An understanding of the voluntary and statutory sectors, with the ability to travel across the local area when required.
