Salary: £27126 - £29193 (salary dependent on skills, experience and knowledge)

Base: Wokingham/West Berkshire
Community Connectors are embedded within NHS Community Mental Health teams and support patients to access a wide range of community services and resources. This is including social care, housing, family, debt and employment counselling that support maintaining good health and wellbeing. We are looking for a candidate with experience working within mental health / wellbeing environments, who believes in the benefits of working alongside individuals to enable them to identify their own goals and promote independence and recovery.
