Role Description
Title: Access Wellbeing Co-ordinator
Responsible to: Senior Wellbeing Co-ordinator
Time Commitment: 37.5 hours (full time)
Salary: £26,189 per annum
Location: Based in North Dorset, covering the towns of Blandford, Shaftesbury, Sherborne, Gillingham and Sturminster Newton,
Enhanced DBS Check: Required for this role
Context & Purpose:
Dorset Mind are working with partners, to deliver wellbeing hubs across Dorset. This is part of the universal offer of wellbeing support, an element of the mental health transformation programme for adults. The hubs will support people to take the steps that matter to them, by connecting them to what they might need, at the right time and in the right place, ultimately empowering people to make their own choices and moving towards full citizenship within their communities, having a voice, a sense of belonging, purpose, identity and equal access to the rights and opportunities that should be available to all.

Key Responsibilities & Duties
Creating a warm and welcoming first point of contact at the hub, building rapport and relational safety to create a space where people feel safe and welcome
To consider the person as the expert on themselves, employing a strengths-based approach, acknowledging the strengths and skills an individual brings, supporting selfmanagement while recognising that capacity for this may change over time
Enabling and supporting choice, making decisions collaboratively with the person and being guided by their preferences. Advocating for the person at multidisciplinary/interface team meetings and collaborative triage, knowing when to escalate concerns
Promoting the range of local resources, groups and opportunities to encourage positive wellbeing experiences within the community and to support warm transfers to relevant service encouraging the person to advocate for themselves and act as independently as possible
To develop an ‘Individual Learning Plan’ for the person to access Recovery Education courses through the Recovery Education Centre
To co-produce a ‘My Wellbeing Plan’ (or an alternative if individual prefers) collaboratively with individuals, agreeing goals that are important to them
To co-create safety plans, with a focus on safety rather than risk, which support the individual to utilise their personal, community and service resources to maintain their safety
To provide support through face-to-face conversations, virtual conversations, phonebased conversations, on a one-to-one basis as well as in group settings
To use a networked approach to support (using the principles of Open Dialogue- training will be provided) to support communication with a person’s network, and facilitate support to families, carers, and others as appropriate and while respecting the person’s preferences around confidentiality
To be able to effectively communicate any care planning, risk or safety or safeguarding concerns to colleagues in particular when participating in the virtual MDT
To develop an understanding and response to health inequalities of communities which the hub serves. To have effective strategies to engage with people who have been underserved by health and social care
Support and enable local health, social care, and voluntary sector professionals to make appropriate referrals to the service, through building relationships, networking, and partnerships
Accurately record and collect relevant information on record systems, seeking consent and collaboration from the person where possible
To effectively manage caseloads and appointment scheduling, with consideration to modelling self-care and wellbeing
To promote active citizenship, focusing on what a person has to offer (note communities are not just geographic but can also be communities of identity), encouraging them to participate and contribute both within their communities and to the service
To engage with local communities and community groups to build relationships and build understanding of how to provide a supportive environment for people with mental health challenges
To commit to ongoing reflective practice and supervision
To support service users with significant emotional distress and be exposed to hearing about distressing situations
To engage in ongoing learning and development at an individual and at service level
To maintain professional conduct with clients and professionals at all times, modelling healthy boundaries
Be able and aware of social and wider determinants of health and models such as health and digital literacy and supporting behaviour change
To understand the value co-production and lived experience and be able to effectively work alongside clinicians, colleagues in the VCSE sector and those intentionally employed to use their lived experience
To ensure regular review of risks and issues that could impact on delivery
To work within the values, ethos, and vision of employing organisation
To contribute to the service’s marketing, promotion, and publicity
To work in accordance with the policies and procedures and all relevant legislation
To undertake any other duties as required, appropriate to the post
