The Nelson Trust, in partnership with Connect, is dedicated to providing comprehensive community support to women facing substance misuse and additional complex needs. Our holistic and gender-responsive approach ensures that women receive the highest quality of care, advice, and intervention.
As a Women's Recovery Coordinator, you'll play a crucial role in delivering high-quality, gender-responsive support to women experiencing substance misuse and multiple complex needs. You'll manage a caseload of 25 women, providing holistic, person-centred care and interventions to help them achieve their recovery goals.
Key responsibilities:
- Provide one-on-one support with practical issues such as finances, housing, and employment.
- Develop and implement comprehensive, strength-based recovery plans.
- Conduct thorough assessments of client needs across various domains.
- Collaborate with partner agencies to ensure effective multi-agency working.
- Offer bespoke interventions in both group and individual settings.
What we are looking for:
- At least 2 years of experience working with women with multiple complex needs.
- Strong organisational and time management skills.
- Excellent communication and interpersonal skills.
- Knowledge of substance misuse interventions and motivational interviewing.
- Ability to work effectively both independently and as part of a team.
- Car driver with access to a vehicle and willingness to travel.

Groundsperson
For March 2026, an opportunity has arisen for a new Groundsperson to work on the outstanding sports grounds at Cheltenham College. Joining an excellent Grounds team, you will work on some of the best sporting facilities in the country. Work will be varied and will cover all aspects of grounds work.
Experience in grounds work, especially cricket, would be an advantage. NVQ Level 2 in Sports turf would be desirable, as well as spraying and chainsaw qualifications. You must be able to work as part of a small dedicated team. Full UK driving license is also essential.

Who will you be supporting?
We support a range of individuals with a range of different needs and abilities.
In one area of the building we support over 55’s with physical or mental health needs.
On the other side of the building we support under 55’s with mental health.
It is a lovely community that all come together and socialise in the communal areas.
We have 70 apartments and there are 66 people that live here the amount of people we support varies depending on care packages at the time at present we support 28 people but we do have some vacancies so this will increase.
You would need to be fully trained in medication administration, moving and positioning, epilepsy, diabetes, nutrition. You will be given full training on this in your induction with us.
What is important to us?
It is important that we are supportive of everyone and their abilities and needs, we promote independence. Some people we support require low level support such as support with meals, medication, cleaning, others can require a higher level of support with personal care and moving and positioning. The aim of the extra care services is to enable people to live independently but with staff on site 24/7. Some people we support can live in their own apartment but need four calls a day supporting with meals, personal care, medication and cleaning, they don’t need a nursing/care home but they do need a higher level of support.
We also support people with their social hours such as going out into the community for days out, meals out, shopping and supporting to medical appointments.
Our extra care services are not secure buildings and people are free to come and go as they please and have visitors whenever they want there are DOLS in place, everyone has their own front door. They all have pendants which are linked to the phones the support team carry and these are responded to as and when people press them for emergency support.
Other important information?
The staff on duty during the day is 3 in the morning and three in the afternoon and two waking night staff. There are no commissioned calls through the night you are there to respond to any emergency pendant calls. We also have an on call service for community integrated care and also each housing provider has an emergency out of hours contact.

By joining Chest Heart and Stroke Scotland (CHSS) as a Self-Management Training Facilitator, you can be the difference between people just surviving and really living.
You will be part of Scotland’s leading charity providing support to people with chest, heart and stroke conditions to live life to the full again. Our Community Healthcare Support Services form a nationwide network of local services, including our Supported Self Management programme, community support groups, nurses, volunteers and one-to-one support teams helping families adjust to life with a heart or lung condition or after a stroke.
We are looking for a Supported Self Management Training Facilitator to join our amazing team in the Borders area. This role is focused on making sure that people affected by chest, heart and stroke conditions have access to our evidence-based Chronic Disease Supported Self Management programme, enabling people with our conditions to live with, and beyond, their new reality.
Through delivering our Supported Self Management programme, you will support people to develop the skills, knowledge and tools to confidently self manage their health condition and live as independently as possible. You’ll also help people to get back to doing the things that are important to them - from simple things like walking to their local shop following a stroke, to returning to a sky-high hobby of gliding!
You will co-facilitate the delivery of our 6-week Chronic Disease Self Management workshops with another trained facilitator and line manage any volunteers who support the delivery of these sessions. You will lead and motivate participants within a group setting to take control of their lives in a positive way, raising their confidence and empowering them to develop the skills to set and achieve goals, make strong and supportive connections with their workshop peers and make meaningful changes to their lifestyle to support their ongoing self management journey.
Candidates don’t need to have medical knowledge, or previous experience as a Chronic Disease Supported Self Management Facilitator, as full training is provided. We are looking for someone with a positive attitude towards people with disabilities and long-term conditions and an understanding of the challenges faced by people with chest, heart and stroke conditions and Long Covid. You should have excellent communication and interpersonal skills and an ability to engage with others confidently and deliver key information, clearly.

Main job responsibilities:
- Build trusted one-to-one relationships with individuals who need support with their health, wellbeing and confidence.
- Use motivational interviewing and coaching techniques to empower people to take control of their wellbeing and make positive lifestyle changes.
- Complete assessments using Warwick Edinburgh and WOOP plans to improve participant motivation and engagement.
- Support customers to access internal and external services in order to support their needs to help achieve a positive outcome.
- Support customers to maximise their health through take up of non-acute, non-medical community-based services.
- Co-design individual and small group health and wellbeing activities to improve participant outcomes and reduce health inequalities.
- Manage a caseload and maintain accurate records, progress notes and documentation in line with GDPR and safeguarding requirements.
- Using an agreed case management system, support people to set realistic goals to achieve improved health and wellbeing outcomes
- Attend and contribute to national social prescribing worker events to keep up to date with best practice and training opportunities
- Upholding safeguarding responsibilities and embedding good practice at all times
We're looking for someone who has:
- A relevant qualification or experience of working with vulnerable people, social housing customers who present with health inequalities and social exclusion risk factors.
- Experience of using evidence-based assessment tools such as Warwick Edinburgh or the ability to learn and utilise this tool.
- Experience of case management
- An ability to complete face to face assessments in a variety of environments including home and community settings.
- An understanding of the social determinants of health and the ability to suggest interventions to reduce their impact.

The role of a Support Worker:

JOB DETAILS Job title: Darlington Volunteers – Volunteering for Health Coordinator Grade: Hospice Band 3 Reports & is responsible to: HR Manager Location: St Teresa’s Hospice 2. JOB PURPOSE Main purpose: To develop, set up and deliver the activities of the Darlington Volunteering for Health Hub based at St Teresa’s Hospice, on behalf of the Darlington Volunteers partnership. This post will focus on developing infrastructure to maximise the potential of volunteering within the local health and social care sector, including support with recruitment, selection and compliance checks and centralised induction. Further information: Darlington Volunteers is a funded partnership between lead partner Citizens Advice Darlington, Redcar and Cleveland (CADRC), Darlington Association on Disability (DAD), St Teresa’s Hospice and County Durham and Darlington NHS Foundation Trust Charity. The main aims of the partnership are to: increase volunteer diversity develop local volunteering infrastructure use co-production and support to minimise barriers and increase volunteer voice develop, build and grow NHS Charities County Durham and Darlington Hospital Discharge Programme. Darlington Volunteers is funded as part of the national Volunteering for Health programme that is being delivered in partnership by NHS England, NHS Charities Together and CW+, the official charity of Chelsea and Westminster Hospital NHS Foundation Trust. It is being designed to deliver the recommendations of the NHS Volunteering Taskforce published in June 2023.
