Community Integrated Care supports individuals with a wide range of needs and in many cases, people whose needs and histories mean that individuals can reasonably be predicted to present with behaviours that challenge.
The organisation’s current approach to significant behaviour is to use the least restrictive practices to support the individual through stressful episodes and as such the relevant colleagues are trained in the MAPA (Managing Actual and Potential Aggression) model of interventions (licensed through the Crisis Prevention Institute). However, the aspiration is to for the organisation to adopt the recovery model approaches of Active Support and Positive Behavioural Support which advocate a more proactive approach to prevention and reduction of behaviours that challenge and overall improvement to quality of life outcomes.
The successful candidate will possess excellent writing skills, as well as strong verbal communication skills. Furthermore, the candidate should also possess strong computer skills (e.g., formatting Word documents, creating Excel spreadsheets), and should have experience in conducting research. Additionally, the candidate should have knowledge of care standards and social care legislation. A knowledge of Microsoft SharePoint is preferable but not essential as this can be trained.
You will have experience working with multi-disciplinary teams and have excellent communication and interpersonal skills with internal and external agencies

Known within Avalon as a Support Co-Ordinator this role is better known in the Social Care Sector as a Senior Support Worker. This exciting and varied role will suit someone who’s passionate about providing outstanding care and wants to develop their management skills.

Job Description

We are looking for a dedicated team to deliver person centred and bespoke trauma informed support to vulnerable women in Luton. This new service will provide a safe, secure, and nurturing environment for women facing sexual exploitation and substance dependency – a place they can truly call home. Through stable, person-centred care, you'll play a vital role in empowering women to rebuild their lives and embrace brighter futures.
In this role, as a Support Mentor, you will work within a 24 hour service which provided person centred support to our residents, working to ensure the safety and wellbeing. Our residents have experienced multiple forms of exclusion, such as historical/ongoing abuse, involvement in the criminal justice system, mental health challenges, drug and/or alcohol dependency and enduring high levels of violence and coercion. You will build trusting professional relationships with residents, helping them to achieve their goals and aspirations which could be through tailored one to one sessions, group sessions, in and out of service activities, and general wellbeing check ins. Your role will focus on empowering our residents to take control over their lives and meet their personal goals
Responsibilities include:
- Holding a caseload, conducting key work sessions which are personable to resident needs

Under this umbrella, the partnership delivers an adult substance misuse service for the Bradford District. We provide a safe environment for people to develop life skills and confidence, ultimately enabling them to rebuild their lives and become active, contributing members of the Bradford community.About the role...We are looking to appoint Sessional Recovery Coordinators on a fixed term basis, under the New Vision Bradford umbrella to work as part of a team to provide collaborative recovery planning and case management to substance misusers (drug and alcohol) across all stages of the individuals’ recovery journey, from assessment and engagement through to sustained recovery.Other duties will include:
- Manage an adult caseload of a variety of substance use issues, opiate/non opiate and alcohol
- Devising, implementing, and reviewing ongoing recovery and treatment plans.
- Facilitate and promote individuals’ progression from initial engagement, through structured treatment, to self-sustained recovery.

We are looking for a Support Worker to join our Supported Living Services based in Chippenham on a Full time or Part time basis.
As a Support Worker, you'll have the opportunity to make a real difference in the lives of individuals with learning disabilities. You will be working with a diverse group of people, each with their own unique abilities and needs.
From assisting with daily activities to providing emotional support, you’ll play a vital role in their life.
We support a variety of people with different interests from music, craft, cooking, gardening, allotment work, local walks, board games, pool and darts. The most important thing to the people we support is having someone kind and caring that they are able to spend time with a have a chat.
What we're looking for:
