Introduction to the role: We are seeking passionate, caring, and dedicated individuals to join our team as Outreach/24/7 Support Workers in Brighton and Hove. If you are committed to making a real difference in the lives of people with learning disabilities and autism, we would love to hear from you. In this role, you’ll be supporting individuals in developing their independence, building on their strengths, and leading a fulfilling, active life within their community.
As a Support Worker, you’ll provide tailored care and support to individuals in both their homes and the community. Your role will focus on helping people live as independently as possible, beyond daily living tasks you may be supporting individuals to build on self-esteem, personal growth, and active participation in social and community activities.
Brief outline of the role: The role will be to support them to be as independent as possible with all daily living tasks. You will be required to support them with their regular routines and activities, maintain their tenancy, learn new skills around cooking and support with personal care and medication.

7. Main Purpose of the Job:
The Development Worker will oversee work that meets the Lancaster District Food
Justice Partnership (LDFJP)’s charitable aims and future action plans. You will have a
passion for engaging with communities around food insecurity and issues of
poverty. You will have excellent organisational skills, the ability to work with a range
of people, work well in a team and use your own initiative. Experience of planning
and delivering community-based projects is essential for this post.
The overall aims are:

Increase involvement in decision making around anti-poverty work and food
insecurity, by people with direct lived experience

Main Duties & Responsibilities
Specific Responsibilities
● Work with LDFJP members, and the wider FoodFutures network, to further the
charitable aims of LDFJP, ensuring member organisations are sharing
information and working in a collaborative way
● Consult and engage with relevant communities and partners to ensure
co-development of solutions that appropriately meet the needs of our target
audiences
● Specific support of LDFJP food club network. This includes organising,
facilitating and chairing monthly meetings with the LDFJP’s food clubs,
regularly visiting food clubs when needed, liaising with FareShare and
Lancaster City Council regarding distribution of HSF (Household Support
Fund)
● Work with partners to maintain and develop appropriate monitoring and
evaluation and complete data collection. Ensure appropriate measures are
taken to record impact
● Promote improved understanding of broader support available to
communities
● Research possible training opportunities for staff, volunteers and
communities
● Work with partners to identify gaps in provision and development
opportunities and support with funding applications where necessary in
completing actions

About this role…
We are looking for a Lead Worker to join the City Outreach Team.
The Thames Reach Outreach teams operate across London and help those who are rough sleeping to find routes away from the street. The teams respond rapidly and work flexibly to reach people. Individuals are assessed to identify their support needs and support plans are created, including work to obtain identification, access health services and refer to other specialist agencies, including those offering immigration support.
We will not accept applications where Artificial Intelligence (AI) has been used to assist with completing the competency questions.
You will:
- Work with the Senior Practitioner and Lead Manager to ensure that support provided to people sleeping in encampments is effective and assertive.
- Lead regular outreach shifts, assessing new clients and quickly building trusting relationships.
- Manage a caseload of rough sleepers, identifying the quickest and most appropriate route off the street, and devising a plan to achieve this.
- Risk assess clients dynamically and participate in risk mitigations and safeguarding work, particularly within encampment settings.
- Use a variety of databases and virtual tools to accurately record and map tents and rough sleeping hotspots and encampments to ensure our support is targeted and effective.
- Represent the team at multi-agency meetings and case conferences, leading on work around tents and encampments with police and community safety colleagues.
You will:
- Be confident and professional representing the team and advocating for clients in multi-agency meetings with other services that may hold different priorities and ways of working.
- Have excellent communication, interpersonal, and advocacy skills, proven within a context of working with vulnerable adults.
- Have strong time management and prioritisation skills, and the ability to deploy these independently.
- Have experience and knowledge of support strategies to manage a caseload alongside the liaison functions of the role.
- Have a creative response to rough sleeping and be able to work imaginatively with clients to plan their end to street homelessness.
- Have the desire to work in a busy team and collaborate with colleagues to achieve quick but sustainable outcomes for our clients.

Job Type: This is a pivotal and unique role that supports the deliver of the Dad Matters programme in Kirklees, working in partnership with health partners in community outreach locations including clinical environments. The Dad Matters Co-ordinator will deliver a range of universal and targeted interventions aimed at male parents in the first 1001 days. They will support dads and other male carers with attachment and bonding, mental health and access to services.

Essential requirements include:
- Good interpersonal, motivational and communication skills
- Build on the Home-Start foundation of peer support by engaging and supporting dads in one-to-one and group situations
- Ensure that parent – infant relationships are a thread through every relationship and conversation with dads
- Raise awareness of the risks of poor mental health for fathers and their families during the perinatal period and awareness of the impact on fathers of caring for a partner affected by perinatal mental health issues
- Develop close links with health partners and voluntary agencies in Kirklees
