As our Rehabilitation Specialist you will:

Run a series of 12-week rehabilitation programmes.

Facilitate and deliver both live stream and in person group exercise classes, with people of varying abilities and physical function.

Be able to promote self-directed activity using pre-recorded on demand classes and a journal which will aid in the participants being able to self-manage their condition after the 12-week programme.
-
Be able to re-calibrate exercises and intensities based on regular improvements demonstrated by changes in effort and performance.
-
Identify any deterioration of participants and where necessary follow internal escalation processes.
-
Be able to deliver or facilitate various exercise modalities from circuit training to strength training, whilst working within peoples varying physical abilities.
-
Be required to work flexibly and have good organisational skills to balance multiple programme groups (cohorts) at once and process new referrals.

Engage in 1to1 telephone conversations each week with every participant to offer coaching in terms of setting short term and long-term goals, understanding restrictions, monitoring progress, understanding and offering emotional wellbeing support, and empowering self-directed activity.

Ensure that outcome data is accurately captured, which is essential in supporting our Charitable Purpose and tracking and monitoring participants progress.

Respond to the ongoing requirements of the programme to ensure successful delivery of each cohort.

Have experience and an understanding of rehabilitation (desired not essential).

As a Positive Behaviour Support Partner you will believe in our ethos and values, and you will want to make a difference in someone’s life. The primary purpose of the PBS team is to lead on the day to day delivery of Positive Behaviour Support within the service, including the creation, continuous development and implementation of Positive Behavioural Support Plans. This will be carried out through regular assessment and observations of the people we support.
The PBS team work closely with staff teams to provide a consistent person centred approach, to share good practice, advise and upskill in the principles of PBS.
Key Responsibilities
- The timely construction of Positive Behaviour Support (PBS) Plans.
- Review and adaptation of PBS plans in response to the level of need and best practice.
- Gathering information from a range of sources; direct observation, written, verbal and through supportive questioning of the person and support staff.
- Liaison with staff teams across the patch to aid with broader behavioural support needs.
- Support Team Managers to implement PBS plans.
- Work in collaboration with Health and social care professionals involved in the care and support of our individuals.
- Support teams to create their own specific aids for the people we care for such as social stories, augmentative communication aids and sensory support.
- Ability to coach, mentor and act as a positive role model, consistently demonstrating best practice in documentation and recording standards (e.g., incident reports, ABC charts).

- Promote a collaborative team approach at all times.
- Demonstrate politeness, courtesy, and support towards all team members.
- Maintain flexibility in line with business needs, including working hours.
- Be available to provide cover for staff absences (e.g., sickness, annual leave).

Job Description – Service Manager – Mediation and Child Contact Services Overall purpose of role Kapella’s core focus is to build better relationships and improve the wellbeing of families and individuals. We pride ourselves on offering accessible and affordable services, including counselling, family mediation, training and domestic abuse programmes across the wider Birmingham and Black Country area, with self-paying clients as well as funded work through contracts, grants and partnerships. Our Mediation and Child Contact Services aims to help guide families to conflict resolution, support separated parents and provide safe, child focused contact. As the Service Manager, you’ll be instrumental in developing and shaping this service, alongside establishing and overseeing new child contact centre services. This role combines service leadership with operational management, focusing on quality assurance, safeguarding and continuous improvement, and achieving sustainable growth. Main duties and responsibilities Service Leadership and Development • Work with the Chief Executive to develop the current service offering in line with the strategic plan. • Ensure service meets the needs of the individuals, families and children we support and is also responsive to changes in demand, legislation, commissioning requirements or funding opportunities. Operational Management • Manage service delivery, including case management, staffing, scheduling, and referral pathways. • Line manage staff, placements and volunteers, recruiting as needed. • Monitor and evaluate service performance against KPIs, targets and budgets. • Prepare reports for internal use, contracts, funders and regulators.
