Project Description
The project operates across Ceredigion working with individuals who are vulnerably housed or homeless providing housing related support within people’s homes or in the community, with the aim of preventing homelessness and increasing independence.
Purpose of Role
To coordinate and provide support that enables and empowers Service Users to address housing and/or personal difficulties, build their skills and confidence, and ultimately improve their ability to maintain their accommodation. The support will not create or increase dependence. It will ‘do with’ rather than ‘do for’, so it’s likely that support will involve accompanying Service Users initially, in order to familiarise them with an activity or appointment. Support will help people improve their control, understanding and involvement in dealing with the issues that affect them, and assist Service Users to achieve their preferred outcomes.
This role is subject to an Enhanced DBS disclosure.
Please note: Driving Licence and access to own vehicle is essential for this role.

Project Description
The project is located in Swansea and works with long term street drinkers. The service provides residential support with the aim of meeting individuals’ needs, which promote independence and lead to a sustainable move-on.
Purpose of Role
Working within our established Swansea Substance Misuse Services at Swansea Shoreline. You will work directly with clients who are alcohol dependent through harm reduction who may also be under the influence of illicit substances. Experience of working with substance use is advantageous however full training is available.

Project Description
Housing First Anglesey helps homeless people to find a permanent home quickly, providing ongoing support to help them settle in and maintain their new home. The project provides an intensive support package to actively address issues in a creative and innovative way.
Purpose of Role
Responsibility for coordinating and providing a flexible person centred, solution focused service, applying an assertive engagement and positive risk-taking approach, in line with Housing First principles. Within legal, moral and ethical boundaries, Housing First provision focuses on the long-term goal of creating sustainable tenancies and maximising connectivity with the local community. This role will work in an outreach capacity with those clients who may primarily need support with their mental health, enabling clients to access the services and interventions they need to build their resilience, gain confidence, reduce harm and acquire the personal and social skills they need to meet their aspirations.
This post will work specifically with Service Users whose lead support need is Mental Health, which may be accompanied by other complex needs.
This role is subject to an Enhanced DBS check

Project Description
The project operates Swansea working with women who have substance misuse issues and other complex needs, such as domestic abuse. The service provides residential support with the aim of meeting individuals’ needs, which promote independence and lead to a sustainable move-on. The project also provides housing related support within people’s homes or in the community, with the aim of preventing homelessness and increasing independence.
Purpose of Role
To provide residential and floating support that enables and empowers Service Users to address housing and/or personal difficulties, build their skills and confidence, and ultimately improve their ability to maintain their future or current accommodation. The support will not create or increase dependence. It will ‘do with’ rather than ‘do for’, so it’s likely that support will involve accompanying Service Users initially, in order to familiarise them with an activity or appointment. Support will help people improve their control, understanding and involvement in dealing with the issues that affect them, and assist Service Users to achieve their preferred outcomes. In addition, to assist with the housing management needs of the project ensuring the delivery of a safe, secure and effective service.
This role is subject to an Enhanced DBS disclosure.

Neath Port Talbot Prevention and Wellbeing Support provides generic floating support to the residents of Neath Port Talbot. Project Description
Rapid Rehousing is based around the ethos that housing is a human right and differs from traditional supported housing and homelessness provision that places a number of conditions on people to prove that they are ready for independent housing.
Following similar principles set out by the Housing First model, Rapid Rehousing is a housing led approach with CTI critical time interventions to successfully support people with repeat histories of homelessness, who experience multiple disadvantages, into independent and stable accommodation, through the provision of intensive wrap-around and trauma informed support.
The principles of Housing First are:
- People have a right to a home
- Flexible support is provided for as long as is needed
- Housing and support are separated
- Individuals have choice and control (restricted housing choices with RRH)
- An active and assertive engagement approach is used
- People are supported to identify their strengths, goals and aspirations
- A harm reduction approach is used
Purpose of Role
The Rapid Rehousing Worker will be responsible for coordinating and providing a flexible person centred, solution focused service, applying an assertive engagement and positive risk-taking approach, using Housing First principles but with a view to rapidly addressing a person’s housing need. Within legal, moral and ethical boundaries, Rapid rehousing provision relies on staff being committed to the ethos, applying a ‘can do, will do’ approach, that focuses on the long-term goal of creating sustainable tenancies and maximising connectivity with the local community. The Worker will be working in a capacity to enable clients to access the services and interventions they need to build their resilience, gain confidence, reduce harm and acquire the personal and social skills they need to meet their aspirations and sustain their tenancy.
The successful applicant will be expected to:
- Manage a caseload and establish open and trusting relationships
- Deliver support and drop in sessions in temporary accommodation settings including bed and breakfasts
- Deliver trauma informed support
- Advise on, liaise with and facilitate access to other agencies including; Housing Options, Registered Social Landlords, Private Rented Sector, Adult Social Services, Criminal Justice Services and Drug and Alcohol Services.
