Working as part of the Live Well Team the post holder will deliver one to one Live Well Coaching (social prescribing) with a focus on children and young people wishing to access community-based help and support. Using a Motivational Interview based approach, the post holder will support children and young people to set goals to improve their independence and resilience. They will work with young people across a broad range of complex issues including social connectedness, relationships with peers and family, personal safety, education, employment and training, financial support including welfare rights, housing advice and provide help to improve manage and maintain a healthy lifestyle. The post will work closely with Greenwich CAMHS, receiving direct referrals and supporting young people to “wait well”, they will also receive referrals directly from Primary Care and other partners working with young people.

  • Accept referrals for children and young people with health & wellbeing concerns who wish to benefit from community support. Deliver one to one Live Well coaching for these young people. Live Well Coaches will provide support, information, motivation and advice to enable and promote behaviour change through supporting the client to set SMART goals to enable clients to take more responsibility and control of their lifestyle choices and health improvements.
  • Develop strong positive working relationships with key children and young people support services within the borough including but not limited to Children’s Services, Young Greenwich, Start Well, Education and Childcare settings and Health providers.
  • Pro-actively build partnerships and pathways into the service to ensure children and young people in need are appropriately referred into the service.
  • The Live Well coach will be required to work closely with CAMHS, GP practices, local primary care networks and thewider Live Well Coach team.
  • Proactively contact, engage and inspire young people to participate, assessing their needs and offering a personalised approach to include face to face meetings, home visits, telephone / video support, or social media as required. · Live Well Coaches will use their creativity and innovation to signpost individuals to the most appropriate local services to support their clients to resolve complex issues. They will be expected to identify and research the services available in the Borough, who are able to provide specific targeted support that supplements, or supplants the service provided by the Live Well Coaches. For example, CAMHS, Welfare Rights, and First Base Housing.
  • Motivate, empower and encourage young people to take positive action to improve their health and wellbeing, by connecting with others, attending groups, promoting self-care, volunteering, accessing advice and information and support services. Set goals and develop plans with young people to help them take control of their health and wellbeing.
  • Work with young people in a supportive, holistic way to address practical and psychological barriers. Support children and young people to build positive relationships with organisations providing help within the Borough. · Maintain regular, supportive contact to address issues as they arise and ensure children and young people progress and achieve their goals.
  • To actively promote all young people to be offered the opportunity to have a voice, get involved and to influence operational and strategic direction of the service. This includes promotion and engagement with existing participation groups such as GYPC, Children in Care Council, Youth Hub Forums, Young Commissioners and ACE.
  • Accurately record and report the progress of individuals along with statistical information to line manager in the format agreed and on forms developed by the Service and input data directly into IT systems/databases including the bespoke Live Well Data System.
  • Attend reflective practice and supervision, team meetings, objective setting, appraisal and learning activities and line management meetings as required – be involved in the development of good practice.
  • Support and tailor interventions for children and young people with a range of special educational needs and disabilities and social, emotional and mental health needs.
  • Build strong working relationships with internal and external services and to promote link worker/social prescribing model.
  • Liaise with appropriate statutory, private, independent and voluntary sector agencies working with families and build up networks that promote timely access to support for children and young people.
  • Liaise closely with the Family Information Service (FIS) to help promote community provision and support the development of the Local Offer and FIS directory of services.
  • If required attend meetings and/or events to represent and promote the Social Prescribing and Live Well Coach Service.
  • Be flexible – this role will include travelling to other locations to support staff with meetings, client appointments and outreach clinics.
  • Ensure children and young people are made aware of key processes as relevant e.g. advocacy, complaints, information sharing, and to make sure they are asked about how they would prefer information; to ensure special information requirements (e.g. easy read, PECS, large print, language, braille etc.) are clearly recorded on their files and their needs are met.
  • Undertake any other duties as may be reasonably required, commensurate with the role and grade/level within the organisation.
