Purpose of the role As a member of the Linking Leeds team you will make a real difference to the lives of others. You will be a keen advocate of the benefits of supporting people with health and social care needs to access support in their community by linking them to a network of groups and activities, enabling them to have improved health and wellbeing. The post holder will support individuals to build independence and personal resilience, enabling self –management of their health and wellbeing. This will be achieved through the creation of Wellbeing plans which will be tailored to individual need. The role is based over various sites including community venues, GP practices, and occasional work from home. The Linking Leeds office is The Reginald Centre, 263 Chapeltown Road, LS7 3EX. Main Tasks and Responsibilities • To complete comprehensive and holistic assessments of need for those referred to the service within agreed timeframes. • To triage referrals and signpost to specialist support quickly where necessary. • To be part of the serviceʼs Duty Management system, working from the central Hub to support general queries, Light Touch interventions and contribute to the overall smooth running of the service. • To build trusting relationships quickly whilst motivating individuals to make sustained changes, working sensitively and overcoming barriers to access. • To work with a caseload of individuals to facilitate access to recommended community-based interventions and programmes of activity that benefit health and wellbeing. • To develop collaborative Wellbeing support plans with individuals which promote choice and complement any medical treatment or advice. • To identify any barriers to people accessing services and activities and work with individuals to overcome these. • To use a range of techniques and interventions to support people, including those with long term conditions, to self-manage their health and encourage behavioural and lifestyle change. • To monitor and record outcomes within SystmOne through the use of recognised assessment tools and outcome measures, including SWEMWBS and the Wellbeing Wheel. • To build networks with local community resources such as activities and services that may have an impact on health and wellbeing and support Organisation • Participate in 1:1s, performance and development, including Leeds Mind compulsory training programme • Attend and participate in service/team meetings • Work within the framework of Leeds Mind’s policies and procedures • Promote the values, behaviours and ethos of Leeds Mind • Respond and report on safeguarding concerns in line with the Leeds Mind safeguarding policy and procedure • Undertake any other reasonable duties as and when required Person Specification Desirable • A Levels • Degree • Have access to a car for work purposes and hold a full current driving licence and business insurance for your vehicle. • Language skills e.g. Urdu, Polish, BSL, etc. Knowledge/ Qualifications Skills/Ability Essential • GSCE English and Maths (or equivalent) at grade C/4 or above • Full driving licence (consider if this is necessary) • Demonstrate understanding and commitment to equal opportunities and diversity, and to combatting disadvantage and inequality in health provision • High level of IT skills, proficient with a wide range of Microsoft Office programmes (Word, Excel, Outlook, PowerPoint, CRM Database) • Demonstrate a creative, flexible and imaginative approach to supporting and motivating clients with diverse needs to make sustained changes in their lives. • Understanding of Safeguarding processes within the workplace to protect clients and yourself from harm. • Ability to reflect on and share practice with peers. • Good interpersonal skills including listening and display empathy. • Knowledge of a range of community groups and services which support wellbeing. • Knowledge of a range of interventions which support behavioural change e.g. Motivational Interviewing. • Experience • Experience of collaborative working across different organisations to support clients, including experience of marketing to raise the profile of the service to other stakeholders. • Experience of risk assessment in a professional setting. • Experience of working with Client Management Systems such as SystemOne, PARIS, P-MIS, RIO etc. • Demonstrable experience of effective planning and organisational skills to deliver targets to deadlines. • Experience of working with people with diverse mental health and social care needs. • Experience of using recognised assessment tools. • Behavioural • Motivated, confident and • Lived experience of mental resilient. health difficulties. • Demonstrate understanding and commitment to equal opportunities and diversity, and to combatting disadvantage and inequality in health provision. • Lives our values every day – Being Open, Supportive, Brave, Connected, and Resourceful. • Open to change and able to work flexibly in line with the needs of the service/organisation. • Commitment to working in partnership with service users, management, and staff. • Understanding of the challenges faced by people experiencing mental health difficulties. • Demonstrate a commitment to equality and diversity in the workplace. • Commitment to safeguarding clients and others you may come into contact with as part of your role. • Demonstrate a good understanding of and active commitment to anti-racism.
