GP Surgeries within the Coastal Primary Care Network and
Wellbeing Centre (Fareham & Gosport)

29.5 hours per week

 £21,451.28  to  £23,568.11  per  annum  (£26,905  to  £29,560  full
time equivalent) – Solent Mind Band C

To work on a 1-1 basis with young people aged 11-25 years identified by the GP’s across the 3 practices.
This will be overseen by the Wellbeing Manager. These sessions will focus on teaching young people
skills and techniques to better manage mental wellbeing and to build resilience and coping skills.

Principal Accountabilities:

•  To  see  patients  within  the  age  range  of  11-25  years  from  the  three  GP  practices  within  the

network to identify individual needs.

•  To  undertake  assessments,  deliver  1-1  sessions  to  patients  focusing  on  skills  which  help  to

manage feelings of anxiety, low self-esteem and low mood and to build resilience.

•  To record progress on patient records using the EMIS (or equivalent) system in line with the GP
practice/NHS Policy and Procedures and Solent Mind Policy and Procedures, and to collect data
for project monitoring.

•  To  act  quickly  in  response  to  any  safeguarding  issues  or  concerns  and  to  ensure  these  are

reported.

•  To report any concerns around the patient’s mental health to the registered GP or duty Doctor if

appropriate.

•  To contribute to regular monitoring reports for both the PCN and Solent Mind.

•  To  liaise  with  key  partner  agencies,  including  education  establishments,  young  people’s

services, CAMHS.

•  To  network  with  other  agencies  who  may  potentially  provide  support  and  services  to  young

people.

•  To attend relevant meetings/conferences/events and training in line with the duties of the post.

•  To participate in team meetings, supervision and annual appraisal meetings with the Wellbeing

Manager/ Co-Ordinator and to attend Solent Mind meetings as appropriate.

•  To co-facilitate group sessions for young people at the Wellbeing Centre or other venues.

•  To  attend  relevant  training  opportunities  and  workshops  in  line  with  identified  development

needs.

•  To work in line with the project brief.

General:

•  Adhere to the aims, values, policies and procedures of Solent Mind, particularly safeguarding

Qualifications/Education/Training:

•  Educated to NVQ Level 3 or equivalent experience

Experience:

•  Experience of working with Young People

•  Experience of working in a range of different settings

•  Experience of working with colleagues and partners across different settings
