Transitions Facilitator – Maternity Cover
Do you have experience working with children and young people with learning disabilities? Are you skilled at building meaningful relationships with both young people and their families? Do you have the creativity and drive to find the best life solutions for each individual?
We’re looking for a Transitions Facilitator (maternity cover) to play a key role in supporting our students and their families as they move into, through, and out of MacIntyre School and Children’s Homes.
What You’ll Do
Be the first point of contact for families at the pre-placement stage, guiding them through the transition process with care and professionalism.
Support the staff team in integrating children into the school environment, residential settings in Buckinghamshire, Leighton Buzzard, and St Albans, and ultimately into adult provision.
Work closely with the Senior team at MacIntyre school and children's homes and our multi-disciplinary team to liaise with local authorities and ensure smooth transitions to adult services.
Help promote our school and children’s homes, contributing to the development of engagement strategies and generating ongoing referrals.
Keep the focus on the individual: in every interaction, you’ll maintain a person-centred approach, ensuring the child or young person’s needs always come first.
What We’re Looking For
A self-motivated professional with excellent communication skills and experience in supporting young people with learning disabilities.
Someone who can work independently, manage their own diary, and adapt to the needs of families, including occasional evenings and weekends.
A creative thinker with the imagination and drive to find the best solutions for each young person’s life journey.
Working With Us
Mostly daytime hours, Monday to Friday (9:00am – 5:00pm), with flexibility for family needs.
Based at our school in Wingrave, with travel across Buckinghamshire and some neighbouring counties.

About you
You'll have worked with children with learning disabilities.
You'll be someone who can see the bigger picture and have the visionary skill to be able to help develop the service further.
You will need to be flexible and organised, with a positive approach.
You will have a talent to bring people together from across the organisation.
You must be a driver with a manual licence, as you will be driving service vehicles.
Who We Are
At MacIntyre Children’s Homes and School, we provide modern, supportive homes and an innovative learning environment for children and young people. Across six homes, each accommodating up to five young people in their own personalised bedrooms, our experienced staff and home managers ensure every child feels safe, valued and empowered.
Learning and development happen throughout the day - at home, at school, and in the community—through life skills, creative arts, physical activity, and community access. We focus on developing communication, functional skills, independence, and confidence, supporting children to participate fully in everyday life.
Following our recent Ofsted inspection, our homes and school were rated ‘Good’. Inspectors praised how staff value children’s voices, use individualised communication aids, and work with therapists to help children express themselves and make choices about activities, room décor, and meals. They also highlighted the team’s proactive approach to supporting emotional wellbeing, using calming strategies, reducing restrictions, and understanding the reasons behind distress.
We celebrate each child’s achievements and unique strengths, and our teams collaborate to meet new challenges, creating homes and learning spaces that are aspirational, safe, and supportive. You will receive specialist training, including in behaviour management and personal care, and be supported to make a real difference every day.
