Salary – £28, 367.50 FTE (£14,183.75 pro rata)
Hours – Part time, 18.75 hours per week
About the Role
At Longfield Hospice, we believe in helping people live as well as possible, for as long as possible. Our Living Well Service supports patients with a palliative diagnosis to maintain independence, manage symptoms, and improve quality of life through holistic, rehabilitative care.
We are looking for a Therapy Support Assistant to join our supportive multidisciplinary team and play a key role in delivering patient-centred interventions across hospice and community settings.
About the role
Working under the supervision of the Counselling and Living Well Manager, you will support the planning and delivery of therapeutic and self-management programmes for patients experiencing fatigue, breathlessness, anxiety and reduced function. You will work closely with patients, families, carers and colleagues to help individuals achieve goals that matter to them, while promoting dignity, independence and wellbeing.
This is a varied and rewarding role combining hands-on patient support, group facilitation, equipment provision and collaborative working within the wider hospice and community teams.
Key responsibilities
- Support the delivery of holistic rehabilitation and self-management programmes
- Assist with fatigue, anxiety and breathlessness groups and exercise programmes
- Monitor patient progress and report changes or concerns to the Counselling and Living Well Manager
- Support patients with exercise, rehabilitation and use of equipment
- Contribute to goal setting, reviews and multidisciplinary discussions
- Accurately record clinical activity using SystmOne
- Support patients and carers with equipment, adaptations and education
- Promote high standards of compassionate, person-centred care at every contact
- Work collaboratively with hospice, community and external services
About you
You will be a compassionate and motivated individual with experience working in a healthcare environment, ideally as a Therapy Assistant. You will enjoy working as part of a team, communicating effectively with people who may be living with complex needs, and supporting patients through sometimes challenging situations with empathy and professionalism.
You must have access to a car and have a full clean driving licence.
