- Providing practical and emotional support to service users, including welfare checks, phone calls, shopping, and help with day‑to‑day tasks.
- Delivering flexible, reactive support to adults after a hospital stay to enable early discharge and reduce the risk of readmission.
- Responding to referrals from Clinicians and Community Health Professionals, assessing individual needs and creating tailored support plans.
- Assisting with essential activities such as attending hospital appointments, prescription collection, and other tasks that service users may struggle to complete independently.
- Hold a full UK manual driving licence, have access to a vehicle, and be comfortable working flexibly outside typical hours.
- Possess good knowledge of NHS and Social Care services, using this to enhance service quality for users.
- Bring a professional, diplomatic, and confidential approach to handling queries and improving service delivery.
