£10,001 per annum (£12.45 per hour)
Hours
Part-time: 16 hours per week

About the role
This role will be based between our Outreach Services, and Supported Living properties. You will be required to travel between them, it is there fore essential you have access to your own car and a UK Driving License.
As a Support Worker, you will:
- Work in partnership with the team to provide person-centred, outcome-focused support.
- Maintain the dignity, privacy, and wellbeing of each individual, assisting with personal care, health, and hygiene.
- Support individuals in a variety of activities, including sport, leisure, cultural and recreational events, day trips, and holidays.
- Assist with communication, sensory support, or psychological monitoring and interventions.
- Providing quality support which meets the identified needs of each person
- Writing records of support provided
- Ensuring that all relevant documentation, such as support plans / risk assessments, is regularly reviewed; to keep them clear and accurate, to reflect each individual’s support needs and preferences.
- Follow Safeguarding procedures and act promptly on any concerns.
What we’re looking for
We welcome applications from individuals who:
- Have experience as a Support Worker, Healthcare Assistant (HCA), Carer, Senior Support Worker, or Team Leader, preferably with autism or learning disabilities.
- It is essential to hold a full UK manual driving licence.
- Can identify and support the needs, wishes, and aspirations of individuals with communication needs.
- Are experienced in providing intensive behavioural support.
- Have strong communication skills and experience coordinating activities in the local area.
