Job Title: Support Worker - Homelessness Job Location: As per Contract Accountable to: Service Manager Hours of Work: See contract for full details - Flexible over a 7 day rota Key Performance Objectives: • To provide housing related support to service users to enable them to maintain their licence and to achieve their aspirations for independent living. • This job description is generic and applies to all Housing Support Workers. The duties will be determined by the individual needs of the service users being supported. The context in which support is provided may vary according to the service users living environment and may include site based or floating support. Key Tasks: • Participate in the assessment of referrals, ensuring that good practice and equality • of opportunity is followed. Identify service users individual support needs and through consultation and agreement with the service user (and where appropriate other agencies) devise and implement suitable action plans to ensure identified support needs are met. • From the date a service user moves in, ensure that the Move On Plan is used as a focal support for support sessions and, in particular, ensure that former tenancy arrears (if applicable) are addressed from month one. • Carry out risk assessments as and when necessary. • Ensure compliance as per Wiltshire Council contract and Alabaré’s policies and procedures. • Ensure Harmonia is kept up to date to reflect the service user’s current circumstances. • Organise and provide appropriate support for service users leading up to their move on into independent living and liaising with the Resettlement Worker. • Ensure that service users have access to specialist support and, where appropriate, to liaise and work in partnership with relevant agencies and with the agreement of the service user, act as an advocate on their behalf. • Liaise with the Housing Administrator in order to monitor non-payment of accommodation charge and take prompt action in line with the agreed policy and procedure on arrears. • Ensure service users understand their licence agreement, their rights and obligations, and report any breaches to Team Leader/Service Manager. • Ensure that weekly health & safety inspections of service users accommodation take place and that any issues, such as maintenance, damage etc. are addressed promptly and followed through. • Keep up to date with issues relevant to housing and support, welfare benefits and other relevant legislation. 1 • This post may require some lone working. Our lone working policy requires that staff carry a service mobile and adhere to the lone working policy.
