Support Worker
Location: Sandwell (covering West Bromwich, Oldbury, Smethwick, Wednesbury, Tipton, Rowley Regis and surrounding areas)
Hours: 18.5 hours per week, rolling rota (Part of a Job Share)
Salary: £22,932 per annum pro rata (£12.60 per hour)
Contract: Fixed Term Contract Until 31st December 2026
Additional Requirement: Full UK manual driving licence held for a minimum of 2 years and a willingness to drive service vehicles
We are looking for a dedicated Support Worker to join our 7-day hospital discharge service in Sandwell. You’ll work closely with hospital teams and partner organisations to help people return home safely after a hospital stay and maintain their independence.
You’ll be part of a supportive team that includes two Support Workers, two Coordinators, and a Service Manager.
What You’ll Do
- Respond to referrals and complete welfare calls and home visits.
- Attend hospital wards and discharge units to support patients.
- Provide practical and emotional support to aid recovery and independence.
- Assess needs, identify safeguarding concerns, and accurately record case notes.
- Signpost individuals to appropriate services and community resources.
This is a hands-on, community-based role where no two days are the same. You’ll need flexibility and confidence working in different locations across Sandwell.
Shift Pattern
- 4-day working weekon a rolling rota (3 long days + 1 short day).
- Long days:9:00am – 2:00pm or 3:00pm – 8:00pm
- Short day:9:00am – 12:30pm or 4:30pm – 8:00pm
- Includes one weekend day every three weeks: 9:00am – 8:00pm (2 long days + 1 short day on this week)
- Rotas provided 3 months in advance.
