The aims of CTM Substance Use Service are to engage people who use drugs or alcohol, and or their concerned others, to access support to make positive life changes. This includes early intervention, engaging and assessing service users, needle exchange, brief interventions, outreach, training, case management, health promotion and harm reduction initiatives and recovery and aftercare services.

Barod services are provided in a non-judgmental, non-discriminatory, and professional manner and in a way that demonstrates courtesy and respect for individuals using substances, and sensitivity to their unique personal situations and experiences.

To provide community-based alcohol interventions to people who have been assessed in hospital by the Alcohol Care Team (ACT) following alcohol related hospital admissions. Postholders will carry out assessments and interventions predominantly in people's own homes as well as other community-based settings.

This role will work closely with the hospital ACT to ensure appropriate procedures are in place for community discharge, and handover of ongoing treatment needs/recovery.

  • To work alongside ACT colleagues to deliver substance use interventions to people identified as having substance use needs.
  • To collaborate with other agencies, including Cwm Taf Morgannwg UHB, third sector agencies and local authority teams, to provide a holistic integrated substance use service.

  • Conduct initial assessment and care planning and provide access to a range of interventions.
  • To support people within the service holistically to ensure all support and treatment needs are met.
  • Undertake risk assessment and develop and review risk managements plans that seek to mitigate identified risk.
  • Provide structured support to service users experiencing issues with alcohol that do not require referral for relapse prevention medication.
  • To utilise psychosocial interventions, motivational interviewing and cognitive behavioural techniques where appropriate to promote engagement in treatment services, encourages self-esteem, well-being, self-responsibility and enhanced motivation.
  • Understand and use pathways (for detox and rehab services) available to those who use alcohol.
  • To actively engage with service users in the community and work to re-engage them in treatment services if required.
  • Support service users to attend initial appointments or appointments with other health professionals.
  • To work within agreed policies, procedures and protocols to safeguard children, vulnerable adults and those experiencing domestic abuse.
  • To work in partnership with other agencies to provide a package of support around holistic needs, by attending joint allocation meetings and case reviews.
  • Enter data onto the case management system Palbase, and use the system effectively to inform work with service users, and undertake relevant administrative functions.
  • Contribute to the development and review of organisational policies and procedures as and when appropriate.
  • Positively promote and represent CTDAS and BDAS in the public / external arena including engaging with media.
  • Retain and maintain all information (paper and electronic) in line with service level agreed guidelines and statutory protocols, such as GDPR.
  • Enter data onto the relevant case management systems, such as Palbase
  • Enter data onto all other required databases such as NEO
  • Provide training or talks to internal staff or external organisations as required.
  • Support volunteers in the delivery of service provision.
  • Take up arrangements for support and supervision and participate in all relevant internal meetings.
  • Co-operate with requests from CTM substance use management and administration.
  • Work with line manager to meet Key Performance Indicators (KPIs) and other targets set by the commissioning team or targets set internallyOther duties

Level 3 in Advice and Guidance (or equivalent) or willingness to work towards.

Ability to work effectively with people regardless of their ethnic, cultural, social backgrounds, their gender, age, religious belief, disability, and sexual orientation.

Proven ability to network and build professional relationships.

The ability to compile, maintain and complete accurate written records, including data capture and the use of case management systems.

Excellent literacy, numeracy, and ICT skills, with a close attention to detail.

Previous experience of working within the substance use sector or similar field.

Experience of working in a support role within the community.

Understanding of substance use treatment pathways.

Understanding/knowledge of ACEs and their impact.

Experience of a trauma-informed approach.

Experience supporting vulnerable people or families.

Experience of working in an outreach role within the community.

Understanding of alcohol care pathways in the community.

Flexibility to work at multiple locations and flexible working hours outside the normal working week

Ability to problem solve, work under own initiative and able to work under pressure.

Ability to establish and maintain clear boundaries in respect of personal and professional responsibility.

Time management skills – excellent ability to plan, prioritise and manage own workload and time in a fast paced, demanding role

Full, current driving licence and use of a car which is insured for business use is an essential requirement of this role.

Ability to communicate through the medium of Welsh.
