Salary circa £22,600 per annum | 28 hours per week | This is a fixed-term contract until 31 March 2027

We’re looking for an innovative, compassionate and professional person with strong communication and organisational skills to join our Stroke Recovery Service in Herefordshire and Worcestershire.
In this role, you’ll support stroke survivors and their families as they adjust to life after stroke. You’ll help identify their needs, provide personalised information, advice and support, and work with them to achieve the outcomes that matter most to them.
Reporting to the Stroke Association Service Delivery Coach, the Stroke Association Support Coordinator will:
- Support stroke survivors and carers as they move from hospital discharge back into the community.
- Use a person-centred approach to help stroke survivors and carers set goals and access the right support.
- Provide clear, tailored information, advice and practical support.
- Encourage stroke survivors to make informed lifestyle changes that may reduce the risk of further strokes.
- Work closely with NHS colleagues and community partners to improve the lives of people affected by stroke.
This role requires extensive travel across a large geographical locality to visit people at home and in community settings. Candidates must be able to demonstrate how they can meet this requirement of the role.
