An opportunity has arisen for an experienced Occupational Therapist to join our Phyllis Tuckwell multi-professional team within our hospice at home service. This is a new role establishing and leading a small team piloting an approach that will provide proactive planned care and palliative rehabilitation for stable patients at home because “every day is precious.” The support will extend to families and carers.
The role requires skills in holistic assessment, problem solving, prioritisation and decision-making, caseload management while working within the multi-professional team to identify and meet individual needs. It requires a positive approach to change and well-developed planning and organisation skills. Central to the role, is supporting patients and their families to adapt to change and uncertainty; setting meaningful goals to support participation in occupations, manage symptoms and optimise quality of life.
We are looking for someone with strong clinical skills, leadership and service development experience, excellent communication and interpersonal skills, and utilises these to build trust, provide support and education.
Regular clinical supervision and support is provided by Phyllis Tuckwell, and team members are encouraged to develop their skills and ensure evidence-based practice to provide best quality patient care.
The postholder and will be able to work flexibly across our services according to patient needs.

About You:
A successfulSpecialist Occupational Therapistwill have:- Diploma or a degree in Occupational Therapy
- Current registration with the HCPC
- Palliative / end of life care or long-term conditions experience
- Membership of the Royal College of Occupational Therapists
- Car driver/owner
- Excellent interpersonal skills
