South Wales Peer Support Coordinator Salary: £28,000 (pro-rata) Contract: July 2026 – April 2028 Hours: 17.5 hours a week (equating to 2.5 working days) Annual Leave: 30 days (pro rata), plus bank holidays and office closure between Christmas and New Year Pension: 8% (3% employer, 5% employee) Responsible Lead for Fundraising, Communications, Wrap-around support and Wales to: services Base: Hybrid model: remote working, with regular outreach at sites across South Wales Quarterly meetings at Positively UK’s London office (Old Street). Travel will be reimbursed. About Positively UK Positively UK is a peer-led, peer-run organisation founded in 1987. Over 80% of our staff are living with HIV. We were founded by people with HIV, for people with HIV, and that remains at the heart of everything we do. We work collaboratively with the NHS to ensure peer support can be accessed easily in clinics, and we offer peer-led services in the community. Our support is holistic: we address the range of needs people with HIV have, including information about HIV, emotional and practical support including welfare, housing and benefits advice, and connections to community and groups. We train peer mentors to develop the skills to support others living with HIV effectively. We work and campaign to end stigma, and we organise a biannual National Conference of People with HIV. We believe that lived experience is the expertise that makes our work effective. About Fast-Track Cymru (FTCymru) Fast Track Cymru is the only Wales based charity with a focus on HIV, hepatitis, TB and related conditions. We work collaboratively regionally and nationally (and even internationally) to raise awareness whilst combatting stigma. Fast Track Cymru is a collaborative network of Fast Track Regions across Wales working to collectively reach the UNAIDS and Welsh Government targets in all Welsh cities, towns and rural areas across the seven Health Board Regions. In July 2025 Fast Track Cymru became the first Fast Track Nation in the world, with all seven health boards signed up to the Global Fast Track Cities initiative. Each of the seven health board regions has a coalition of interested parties, including clinicians and other health professionals, local authorities, academic researchers, educators, community groups, third sector organisations and people with lived experience all of whom are invested in working collaboratively in Wales. While each region is autonomous, they all feed into the national Fast Track Cymru network to work together and inform best practice. Our work is focused on six main areas: - Community Engagement - Campaigns - Data, Research and Evaluation - Partnership and Collaboration - Policy Development and Influencing - Coalitions and Regional Development We centre the voices of those with lived experience in everything that we do and codevelop every element of our work at every stage. About the project Positively UK, in partnership with Fast-Track Cymru (FTCymru), is establishing a new Wales-wide peer support service for people living with HIV. The service aims to ensure that everyone living with HIV across Wales can access high quality, person-centred peer support in a way that works for them. The service will combine clinic-based, community and online peer support. This will create flexible ways for people to have support that works for them. The service will be delivered by two peer caseworkers (each working 2.5 days a week). The Peer Caseworkers will provide outreach within HIV and sexual health services, while also working across community settings to reach people who may face barriers to care, including stigma, isolation, or rural location. A key aim of the project is to build the skills of people with HIV in Wales to provide high quality per support, that can make a real difference according to the National Standards of Peer Support in HIV. The long-term goal of the project is the creation of a strong network of trained peer workers and peer mentors who can continue delivering support across Wales. Role Description We are recruiting two part-time roles (each 2.5 days per week) to work together in establishing peer support across Wales. These roles will operate collaboratively, with one post based in North Wales and one in South Wales, ensuring strong regional coverage and local connection1. 1 We welcome applications from candidates interested in combining the two part-time vacancies into a full-time post. However, due to the travel requirements across Wales, we anticipate that two part-time roles may offer greater flexibility and effectiveness in delivering the programme. The Peer Caseworker will work together with clinical teams in South Wales, while also reaching out into the community to ensure no one feels alone in their HIV journey. To thrive in this role, you must bring a deep, personal understanding of the impact of HIV and a commitment to support others. You will also have experience supporting people with complex needs, and a passion for peer support, empowerment, and local, community change. Key responsibilities: 1. Case Management The Peer Support Coordinator will manage an ongoing caseload of people living with HIV, including: - Responding to all referrals in a timely and appropriate manner - Registering clients where appropriate - Assessing clients’ needs using a wellbeing assessment tool - Identifying safeguarding concerns and taking appropriate action - Co-creating a wellbeing plan - Signposting and referrals - Advocacy support (e.g., hardship grants, UKBA/NASS letters, Care Act referrals) - Regular follow-ups - Reassessment and planned exit from the service You will be responsible for providing ongoing support, ensuring effective follow-up, and working toward the best possible outcomes for each individual, including: - Emotional support - Information, advice, and mentoring - Setting and supporting clients to achieve their goals - Advocacy on a wide range of issues as defined by client needs. These might include adjusting to a new diagnosis, starting treatment, sharing HIV status with others, sex and relationships, mental health, substance use and Chemsex, ageing with HIV, accessing services and treatment advocacy. 2. Group Activities You will contribute to the planning and delivery of the following online and in-person group activities, in collaboration with staff from Positively UK and FTC Cymru: - Support Core Peer Mentor Training with recruitment and logistics - Recently Diagnosed Workshops - Local Peer Hubs - Pop ups in rural areas across South Wales - Online groups and workshops 3. Monitoring and Evaluation - Maintain accurate electronic records on Salesforce - Use wellbeing assessment tool to identify areas of life for improvement and measure progress 4. Peer Mentor Volunteers - Identify potential peer mentors and support the Volunteer Coordinator in recruiting, training and matching peer mentors with mentees for the provision of mentoring, befriending, and community access support 5. Collaboration and Teamwork - Build relationships with the Health Boards and clinics in South Wales - Establish referral pathways from clinics into the programme - Collaborate with local support organisations (e.g. Umbrella Cymru, Pride organisations, Barod, ), rural community hubs, community health teams, etc.) - attend the project’s steering group meetings - Attend FTCymru’s National Advisory Council - Attend FTCymru’s Regional Steering Groups - Attend MDT meetings in clinics 6. Training, supervision and support - Regular one-to-one supervision by the Positively UK Wales Lead - Local context support by FTCymru’s Regional Development worker and Executive Director - Monthly casework meetings with Positively UK’s frontline staff - Monthly staff meetings at Positively UK - Access to external professional supervision by an independent counsellor - Access to employee assistance programme Person Specification Key: E = Essential D = Desirable Experience Living with HIV, adjusted well to your diagnosis, and open about your status (positive for at least 2 years) Fluent in Welsh Language Experience of providing peer support or advocacy Experience of supporting people to navigate health, social care or voluntary sector services Experience of supporting people with complex needs (e.g. mental health, substance use, housing) Knowledge Strong understanding of the social, economic and psychological issues faced by people living with HIV Knowledge of statutory and voluntary services relevant to people with HIV in Wales Understanding of health inequalities and how they affect communities affected by HIV Awareness of benefits, housing and immigration systems Skills and Approach E / D E D D D D E / D E D D D E / D Strong communication skills — written and verbal Commitment to empowering clients Clear understanding of professional boundaries and confidentiality Ability to manage your own time, caseload and priorities effectively Ability to work collaboratively and contribute to a team Willingness to reflect on your practice and use supervision for development IT skills and ability to maintain accurate records Ability to speak a community language E E E E E E E D Other Information • You must be committed to undertaking all required training (including safeguarding, peer support, and mental health first aid). • The post is subject to an Enhanced DBS check. • This role is only open to people living with HIV in line with the genuine occupational requirement of peer-led support. Terms and Conditions The post will be offered for an initial three-months probationary period. Training in peer mentoring and access to Safeguarding Training and Mental Health First Training will be provided to the successful candidate, and continuation in the role is dependent on successfully completing and achieving peer mentoring core training. Normal working hours 9:30am – 5:00pm, including half–hour lunch each day; however, evening and weekend working will be required. Time off is given in lieu and there are no overtime payments linked to this post.
