The twin objectives of St. George’s Crypt are the relief of hardship and poverty among those living with homelessness, poverty and addiction in the city of Leeds and beyond, and the advancement of the Christian religion for public benefit. Safeguarding Responsibilities:
Enforcing the company’s safeguarding policy.

To understand and implement working standards set by the domestic violence and abuse quality mark documentation.

Job Purpose:
To understand and convey the vision of the service through coproduction and an asset-based community development approach.
To provide flexible access and promote self-management to service users.
Facilitating and delivering engaging group health and wellbeing activities aimed at improving and sustaining individuals’ mental health.
To develop an understanding of delivery across the system and partnership, ensuring people have access to the support they need in as few steps as possible.
To achieve this through the Live Well Leeds model and a commitment to partnership working.
Key Job Role Responsibilities and Main Tasks:
Build positive, respectful and professional relationships with service users, understanding their needs, challenges and strengths.
Carry out assessments and support helping individuals to access the right support quickly.
Work with service users and partners to create and review support plans, focusing on strengths, progress and recovery.
Support individuals with their mental health needs, working alongside the Charity’s supported living projects and external service delivery partners.
Help create a welcoming and inclusive environment, working flexibly across different locations.
Promote equality, diversity and anti-discriminatory practice in all aspects of the role.
Work in partnership with internal teams, external organisations, health services and community groups to achieve the best outcomes.
Monitor service users’ wellbeing and take appropriate action if risks increase or mental health declines.
Maintain professional boundaries, ensuring confidentiality, dignity and GDPR compliance at all times.
Support service users to build independence, confidence and self-management skills.
Encourage and support involvement in activities that aid recovery and personal development.
Contribute to service development and continuous improvement alongside colleagues and partners.
Support service users to access community resources that promote inclusion and independence.
Share knowledge and support the development of colleagues, students and volunteers.
Encourage self-advocacy and advocate for service users when needed.

General:
Participate fully in organisational performance processes, including induction, supervision, performance monitoring and appraisal, in line with the employing organisation’s policies and procedures.
Manage an allocated caseload and maintain files in accordance with the Live Well Leeds Service ensuring all information is recorded accurately and within agreed timescales on the central case management system.
Track each service user’s journey through the Live Well Leeds Service, acting as the primary link and monitoring changes in wellbeing, needs and progress.
Collate, maintain and present service user information for monitoring and reporting purposes, ensuring all records are accurate, up to date and compliant with service requirements.
Promote the work and values of the Live Well Leeds Service and St George’s Crypt to the public, potential service users, referrers and funders.
Provide general information about St George’s Crypt and its services to stakeholders and others who may be interested.
To implement the Organisation’s policies, procedures and practices and, to always comply with the aims of St George’s Crypt; to be committed to and implement Equal Opportunities Policy and to promote this with staff.
Participate in team meetings and other meetings as required.
To ensure information is dealt with in accordance with St George’s Crypt policies around Confidentiality, Communications, Internet, Email and Telecommunications and steps are taken to ensure that confidential information is secure.
Represent St George’s Crypt in a knowledgeable and professional manner at all times.
