Job purpose

A move away from the street should be the beginning of something lasting, not a temporary pause before someone faces homelessness again.

Gray’s Inn Road is a 16-bed short-stay accommodation service at the heart of Camden’s work to make rough sleeping rare, brief and non-recurrent. The service provides people with the stability, space and specialist support they need to recover from the impact of rough sleeping, rebuild confidence and prepare for a safe and sustainable move into longer-term housing.

The Resettlement Worker plays a vital role in turning that period of stability into a genuine route forward. The purpose of the role is to ensure that each resident has a realistic, person-centred pathway out of the service, shaped around their strengths, circumstances, aspirations and support needs. This means looking beyond simply finding an available property and helping people access a home in which they have the best possible chance of settling, maintaining their tenancy and building greater independence.

Working alongside residents, Support Workers, housing providers and partner agencies, the Resettlement Worker helps break down the practical and personal barriers that can prevent people from moving on. Through persistent, creative and compassionate support, the role opens up housing opportunities, strengthens residents’ readiness for independent living and reduces the risk of a return to rough sleeping.

Key accountabilities

Support Planning

Carry out comprehensive assessments with clients, helping them identify their life skills, moveon needs and resettlement goals, and agree person-centred plans to achieve them.

Complete thorough risk assessments and risk management plans with clients and relevant professionals, reflecting their resettlement needs.

Identify, report and follow up safeguarding concerns to minimise risks to clients, colleagues and others.

Attend resettlement meetings to support effective planning, task allocation and a seamless transition for clients.

Information Management

Record client contact accurately and promptly, keeping all relevant files and systems up to date.

Maintain client confidentiality and protect personal data in line with organisational procedures.

Support service evaluation and improvement by gathering client feedback through exit surveys, questionnaires and other methods.

Partnership Working

Work closely with SHP teams, particularly the Opportunities and PRS Access teams, to strengthen clients’ tenancy skills and access suitable move-on options.

Work effectively with statutory, voluntary and community partners to coordinate support.

Build relationships with housing providers to increase access to safe, stable and sustainable accommodation.

Maintain a professional approach that strengthens the reputation of the service internally and externally.

Social Inclusion

Support clients to build independence and participate meaningfully in the service, SHP and their wider community.

Promote accessible ways for clients to share feedback, including meetings, complaints procedures and suggestion schemes, responding positively to their views.

Deliver group or individual activities in response to identified needs and share feedback that helps keep the service relevant and responsive.

Client Use of Time

Help clients identify and access life-skills and independent-living activities that prepare them for moving on.

Support clients who are ready to access suitable employment, education, training or volunteering opportunities.

Health and Safety

Understand and follow responsibilities under SHP’s Health and Safety Policy and relevant legislation.

Work safely and ensure local lone-working and fire-safety procedures are followed.

Support building checks and ensure repairs, hazards and other concerns are reported and followed up promptly.

Housing Management

Support the project’s income by helping to minimise rent arrears and void loss.

Monitor rent accounts and address arrears early with clients.

Ensure clients moving on with arrears have agreed affordable repayment plans in place.

Inform the central Voids Team promptly of upcoming vacancies.

Complete pre-void checks as early as possible before clients move on.

Complete all end-of-placement actions accurately on Inform and ActiveH.

Teamwork and Personal Development

Work flexibly as part of the team, sharing knowledge, supporting colleagues and contributing ideas and solutions.

Take an active role in supervision, performance reviews, training and your own ongoing learning and development.

Miscellaneous

SHP is at discretion to amend your responsibilities and, in addition to these, you may be required to perform other duties as may be required for the efficient running of the organisation.

Technical and professional know-how needed for position

Experience and Knowledge

A demonstrable understanding of the Recovery Model, and the ability to engage and motivate people to move towards independence and inclusion.

An understanding of the private rented sector market and ability to broker safe and affordable Move-on options.

A good understanding of the housing pathway to ensure effective and suitable move on options are offered to clients.

A good understanding of safeguarding issues and the ability to undertake comprehensive risk assessments related to this.

A working knowledge of Housing Management as applied to a residential setting, and/or client’s move on accommodation.

Skills and Abilities

A proven ability to work in partnership with external stakeholders, particularly community mental health services and/or substance use services.

A proven ability to create and deliver SMART Resettlement action plans with clients through high quality risk and needs assessments.

A level of numeracy sufficient to carry out tasks such as budgeting with clients, calculating benefit entitlements and monitoring rent payments.
