We are an independent local charity based in Bosham, Chichester offering a wide range of services to support our patients, and their loved ones, who are living with a life limiting or terminal illness. The work we do makes a real difference to the lives of local people and we support over 330 patients at any one time between our In Patient Unit and Community teams.

Why join us as a Band 5 Registered Nurse?

30–37.5 hours per week

Hours: 9:00am–5:00pm, 7-day service including weekend working on a rota basis (approximately 1 in 4 weekends, depending on service needs)

We’re looking for a compassionate and motivated Band 5 Registered Nurse to join our Community Rapid Response Team.

This is an exciting opportunity to join an innovative service delivering urgent, palliative, person-centred care to patients in their own homes. The team supports admission avoidance, facilitates rapid discharge from hospital, and delivers care through our Community Virtual Ward pilot, helping patients with complex needs receive the right care, in the right place, at the right time.

You’ll be joining a friendly and supportive team with a wealth of experience, where you’ll be encouraged to develop your skills and confidence. We welcome applications from newly qualified nurses and are committed to supporting your professional development.

Working alongside an experienced multidisciplinary team, you’ll provide high-quality nursing care while developing your skills in community nursing, palliative care, and urgent community response.

Being a Band 5 Registered Nurse at St Wilfrid’s

What you’ll do

  • Deliver high-quality, person-centered nursing care in the community.
  • Undertake holistic assessments and develop, implement and review care plans.
  • Provide end-of-life care, symptom management and support for patients with long-term conditions.
  • Work closely with GPs, hospitals and the wider multidisciplinary team to coordinate care.
  • Support patients, carers and families with compassion and professionalism.

Qualifications and Experience

  • Registered Nurse with current NMC registration.
  • Excellent clinical assessment, decision-making and communication skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Palliative care experience is desirable but not essential.
  • Full UK driving licence and access to a vehicle is essential.
  • Relevant degree/ diploma or equivalent qualification
  • Right to work in the UK
