Main Purpose

To provide high quality housing related support to people living with a mental illness to assist them to sustain their tenancies, source new homes where applicable and achieve their potential to move on to independence.

To work as part of a wider multidisciplinary team including the CMHTs, Local Authority, GP and others to maximise the support available to people and the goals they wish to achieve.

Specific Tasks

  • To be flexible and responsive to the needs of people using the service and their families as directed by their person-centered plans/recovery plans.

  • To support people to develop and regain life skills and confidence, to make choices and to retain control over their lives.

  • To promote self-esteem and enable people to express their preferences and make choices and decisions.

  • To enhance the confidence and coping abilities through encouragement and positive feedback.

  • Assist people to source, sustain or move into suitable accommodation.

  • To work with people to develop person-centered support plans to ensure tasks and objectives identified are achieved within agreed time scales.

  • To build upon people’s skills and confidence to empower them to maximise their independence.

  • To work in partnership with other agencies to maximise service delivery and outcomes.

  • Support people to maintain a safe, clean and comfortable living environment within their home.

  • Offer advice and support to enable people to develop their skills for independent living including budgeting, paying bills, attending appointments and exploring adaptations or equipment.

  • Assist people to maximise their income and to manage their personal budgets.

  • Support people to understand their rights and responsibilities under their tenancy agreements.

  • Support and motivate people, encouraging them to achieve personal goals.

  • Carry out regular risk assessments and ensure it is properly recorded and kept up to date ensuring risk of harm is reduced as far as possible.
  • Enable and support people to deal with conflict and disputes with others including neighbors.

  • Assist in the delivery and development of activities.

  • Keep all records accurate and up to date to ensure the service can be audited and comply with its contractual obligations.

  • To promote a healthy, active lifestyle and to give person-centred advice and support in respect of diet, exercise, stress reduction, smoking cessation, weight loss and the management of long term health conditions.

  • To inform the Manager and relevant agencies of any concerns or significant changes in the needs and circumstances of service users. To report any concerns regarding children vulnerable adults or children with immediate effect to the Manager.

  • To support people in complying with prescribed medication and self-medication programmes.
  • To liaise with GP's in respect of compliance with prescribed medication including reporting side effects related to medication.

  • Build links with external organisations, to maintain effective working relations and to ensure peoples needs are met.

  • Participate in office duty arrangements.

  • Provide cover for absent colleagues as directed.

General Tasks

  • Attend and participate in training sessions or activities as required.

  • Participate in team meetings and supervision as required.

  • Perform other such duties as may be required to further aims of Target Housing within the Health and Safety requirements.

  • Adhere to all of Target Housing’s policies and procedures in particular our Equal Opportunities, Confidentiality and Health and Safety policies.

  • Carry out other such duties as directed by senior managers as appropriate to the aims of Target Housing.

Person Specification

  • In-depth and up to date knowledge of Welfare Benefits System.

  • Experience in risk assessment and support planning

  • An understanding of relevant legislation relating to vulnerable adults including those with a mental illness or experiencing mental distress.

  • Experience of working in partnership with Local Authorities and Health/Social Care professionals

  • NVQ Level 2 in Health and Social Care is preferred

A Typical Week

You will be allocated a casework of specified people who you will support to achieve their goals. These goals can be varied but may include sourcing alternative accommodation, address debts and develop budgets to aid better money management.

We have offices in each area where you can collaborate with other colleagues and managers. You can discuss your caseloads needs and issues as support for yourself and others.
