You will manage a caseload of approximately 8–9 beneficiariesat any one time, delivering tailored Positive Pathways Plans that focus on developinglife skills, sustaining tenancies, improving wellbeing, and preparing individuals forlong-term independence.Alongside direct support work, the role includes property oversight responsibilities,ensuring supported housing remains safe, compliant, and well-maintained. Theworker will regularly visit properties, monitor condition and occupancy standards,and respond promptly to maintenance or compliance issues.The Positive Pathways Worker will work collaboratively with the Positive PathwaysLead, other Positive Pathways Workers, and internal teams to ensure smoothtransitions through the housing pathway and positive long-term outcomes forbeneficiaries. The role will also provide occasional cover and support for colleagues’caseloads during periods of leave or absence.6Main duties and responsibilities 1. Beneficiary Support and Case ManagementProvide structured, person-centred support to beneficiaries transitioning fromour Crisis Intervention team.Hold a caseload of approximately 10–12 beneficiaries, supporting them throughthe Positive Pathways programme for up to 15 months.Work collaboratively with each beneficiary to develop a Positive Pathways Plan,identifying clear goals that promote independence and long-term stability.Support beneficiaries to develop practical life skills including: budgeting andfinancial management; tenancy responsibilities; maintaining a safe and cleanhome; accessing healthcare and essential services; employment, training, orvolunteering opportunities, and; building healthy routines and relationships.Monitor progress against agreed plans, holding regular review meetings andadjusting support where necessary.Promote accountability, responsibility, and positive behaviour while maintainingclear professional boundaries.Prepare beneficiaries for transition into fully independent housing, ensuring theyare equipped to sustain tenancies long-term.2. Property Oversight and Housing ManagementTake an active role in maintaining the quality and safety of our supported housingproperties.Conduct regular property visits and inspections to ensure accommodation is wellmaintained and compliant with housing standards.Support beneficiaries to understand and meet the expectations of theiroccupancy agreements, promoting respect for the property and shared spaces.Identify and report maintenance issues promptly, liaising with the PropertyManager or relevant contractors where necessary.Monitor property condition to prevent deterioration and protect organisationalassets.Ensure properties remain safe, compliant, and suitable for supportedaccommodation.Complete minor repairs as appropriate.73. Risk Management, Safeguarding and ComplianceMaintain awareness of potential risks associated with supported housing andvulnerable individuals.Monitor safeguarding concerns and follow organisational safeguardingprocedures when required.Conduct and maintain risk assessments for beneficiaries and properties.Work with external partners to manage risk and support positive outcomes.Ensure compliance with organisational policies including: safeguarding; healthand safety, data protection, and; equality and diversity.Maintain accurate, confidential, and timely records.4. Team Collaboration and Service DeliveryWork closely with the Positive Pathways Lead and other Positive PathwaysWorkers to deliver a coordinated and effective service.Contribute to team discussions, case reviews, and reflective practice sessions.Provide occasional cover for colleagues’ caseloads during periods of leave toensure continuity of support for beneficiaries.Participate in training, team meetings, and organisational development activities.Contribute to a positive team culture aligned with the organisation’s values.6. Administrative and Other DutiesMaintain accurate notes, reports, and documentation required for servicedelivery and organisational reporting.Contribute to monitoring outcomes and demonstrating the impact of the PositivePathways programme.ScopeThis role description is intended to provide a guide to the general duties andresponsibilities of the post.
