About the Role

This role includes working closely with patients, families and carers, supporting informed choice, goal setting and self-management and acting as an advocate where appropriate.

Strong assessment, clinical reasoning and care planning skills
Excellent communication and interpersonal skills, including working with complex and sensitive situations
Experience working effectively within multidisciplinary teams
