Main duties of the job
- Providing person-centred support within the community
- Supporting individuals transitioning from hospital to home or supported placements
- Assisting individuals experiencing homelessness with housing, routines, and accessing services
- Supporting people to attend appointments, manage daily tasks, and stay safe
- Building trust with individuals who may have experienced trauma or instability
- Lone working in the community following robust safety procedures.
- Completing risk assessments, support plans, and accurate documentation

Detailed job description and main responsibilities
As an outreach Support Worker, you will provide high quality 1:1 person centred care and support to our clients to enable them to live their best life. You will be passionate in making a difference to our client’s lives, helping them to fulfil their aspirations and ambitions and live as independent as possible.
Skills & Experience Needed:
- Experience in community support, mental health, or homelessness services
- Strong lone-working confidence and decision-making skills
- Consistent professionalism and strong boundary management
- Ability to adapt communication to diverse needs
- Problem-solving skills and the ability to work independently
- Empathy, resilience, and a motivational approach
- Strong safeguarding knowledge and the ability to respond appropriately to risk

- Must have relatable Experience in Mental Health, Substance Use, Learning Disabilities and Autism
Qualifications
Essential criteria
- Care Certificate or required to undertake Care Certificate
Desirable criteria
- Current driver licence
