Job purpose
Rough sleeping pathways only work when people are not left waiting, drifting or falling through gaps. The Outreach Housing Pathways Coordinator is responsible for helping people sleeping rough, or at risk of returning to the streets, move into and sustain the right accommodation through Camden’s homelessness pathways.
Working as part of Camden’s integrated Outreach and Hub Service, the post holder will hold a clear overview of where people are in their housing journey and what is blocking progress. They will work across street outreach, the hub, pathway accommodation, community safety partners and statutory services to remove barriers, progress housing options and ensure agreed actions are followed through with pace and accountability.
The role will focus on the points where accommodation pathways are most likely to break down: delays, exclusions, unsuitable offers, unresolved risks, street activity linked to accommodation, or support needs that are not being met. The post holder will work closely with Outreach Workers, Navigators and accommodation providers to keep plans moving, respond to concerns early, and support people to stabilise before placements reach crisis point.
This is a role that requires persistence, practical judgement and strong partnership grip. Working within Camden’s No Wrong Door model, the Outreach Housing Pathways Coordinator will help ensure that people experiencing complex and multiple disadvantage receive a joined-up response that does not give up when engagement is difficult, risks are high or pathways become stuck.
The purpose of the role is to make sure accommodation pathways are not just processes people are referred into, but routes that actively support people away from rough sleeping and towards greater safety, stability and independence.
Key accountabilities
Housing Pathway Coordination
Coordinate access to Camden’s Adult and Young People’s Pathways, ensuring eligible individuals move efficiently from rough sleeping into appropriate accommodation.
Support referrals between pathway projects and accommodation providers, ensuring placements are timely, appropriate and sustainable.
Monitor cases where tenancy sustainment or behaviour may place accommodation at risk, working proactively with hostels and partners to prevent eviction and return to the streets.
Maintain oversight of pathway “holding spaces” and transitional placements to ensure effective use of accommodation resources.
Integrated Rough Sleeping Response
Work closely with outreach and hub teams to support rapid progression from street engagement into accommodation pathways.
Provide assertive engagement with individuals who may be reluctant to access support or accommodation.
Ensure pathways are accessible to individuals with complex needs, including those experiencing substance use, mental ill health or multiple disadvantage.
Support coordinated responses where rough sleeping and street activity intersect with accommodation provision.
Partnership and Community Coordination
Work with housing providers, hostels, community safety teams, police and voluntary sector partners to address street activity linked to pathway accommodation.
Contribute to Tasking and Targeting forums and partnership meetings to develop coordinated responses for individuals at risk of enforcement action.
Facilitate community engagement where required, supporting dialogue between residents, services and accommodation providers.
Act as a key point of liaison between accommodation providers and the wider rough sleeping service.
Safeguarding and Risk Management
Ensure safeguarding concerns are identified and escalated appropriately in line with organisational and statutory procedures.
Work collaboratively with safeguarding partners to support individuals with high levels of vulnerability.
Support the development of coordinated risk management plans where individuals present complex behaviours or risks.
Promote trauma-informed and psychologically informed approaches to engagement and intervention.
Case Coordination and Practice Excellence
Provide coordinated case management for individuals with complex housing pathways or accommodation challenges.
Apply motivational and strengths-based approaches to support engagement and behaviour change.
Contribute to practice discussions and service development within the integrated outreach and hub service.
Maintain clear and defensible decision-making in line with service protocols and professional standards.
Data, Recording and Performance
Maintain accurate and timely case records using CHAIN and internal systems.
Monitor pathway outcomes, including accommodation access, sustainment and move-on progression.
Contribute to reporting and service improvement through data analysis and performance monitoring.
Ensure documentation supports accountability, safeguarding and contractual reporting requirements.

Experience and Knowledge

Knowledge of Camden’s homelessness and housing landscape.
Skills and Abilities
