Salary: £16,500 (pro-rata) per annum
Hours: 21 hours per week (Job share)
Responsible to: Head of Information & Advice
Employed by: Age UK Hillingdon, Harrow and Brent
Contract: Permanent
Location: Townfield Community Centre, Townfield Square, Townfield Road, Hayes, UB3 2EL (map) and Woodlands Centre, Hillingdon Hospital, Pield Heath Road, Hillingdon, UB8 3NN (map)

To provide wellbeing support to older people with mental health conditions on wards following discharge and those living within the community. Working with partner organisations to focus on supporting discharge as well as those referred by the community mental health team for those living in the community. Reducing risks of readmission and tackling some of the wider health determinants that impact wellbeing.
